摘  要:【目的】探讨MRI水激励(selecitve water excitation,WATS)技术在评价军事训练性髌软骨损伤中的临床价值,以提高对MRI水激励技术的认识。【方法】对10例正常人及8例军事训练性髌软骨损伤患者的MRI影像学表现进行分析,重点观察髌软骨及软骨下骨髓的改变。所有受试者均采用轴位三维快速梯度回波水激励序列(3D-FFE-WATS)扫描。【结果】3D-FFE-WATS序列能够清晰显示髌软骨结构,正常髌软骨可见3层结构。8例髌软骨损伤患者的MRI表现主要为髌软骨全层缺损(5例)或软骨部分缺损(3例),所有患者均伴有软骨下骨挫伤,其中4例伴关节积液,1例伴髌骨外侧脱位,1例伴关节腔游离体。【结论】轴位MRI 3D-FFE-WATS显像可以清晰显示关节软骨的结构,对于髌软骨损伤的检出率高,可用于临床常规检查。[Objective] To explore the clinical value of water excitation technique in the assessment of military training related patellar cartilage injury. [Methods] The MRI changes of cartilage and subeartilage bone were analysed retrospectively in 10 normal subjects and 8 cases of military training related patellar cartilage injury. Axial MR scanning of knee was performed in all subjects, using 3D-FFE-WATS. [Results] The structure of patellar cartilage was clearly displayed on MR images using 3D-FFE-WATS. On 3D-FFE-WATS images, normal patellar cartilage showed 3 laminae. The abnormal MR appearances of 8 patients with military training related patellar cartilage injury showed totally defect of the cartilage in 5 patients and superficial defect of the cartilage in 3 patients. Subcartilage bone contusion was found in all eases. Joint effusion was observed in 4 eases. Corpus liberum and lateral patellar dislocation was found in 1 case, respectively. [Conclusions] 3D-FFE-WATS, axial MRI scanning can clearly display the structure of normal patellar cartilage. It has high ability in finding the cartilage damage of the patellar, therefore 3D-FFE-WATS can be used in routine clinical patellar cartilage examination.
