Preventive Effect of Simvastatin Combined Alendronate on Steroid-induced Osteonecrosis of Femoral Head in Rabbits

摘  要:目的:探讨联合使用辛伐他汀和阿仑磷酸钠对兔激素性股骨头缺血坏死的预防作用。方法:健康成年日本大白兔40只随机分为四组。A组:肌注地塞米松(2.5mg/kg.d);B组:肌注地塞米松(2.5mg/kg.d)和口服辛伐他汀(20mg/d);C组:肌注地塞米松(2.5mg/kg.d)、口服辛伐他汀(20mg/d)及阿仑磷酸钠(10mg/d);D组:空白对照组肌注生理盐水。分别测定用药前(0周)和用药后4、6、8周的血清胆固醇、甘油三酯含量。于用药后第4、6、8周分批处死后行X线摄片及组织学观察。结果:A组中血清胆固醇、甘油三酯含量显著高于其它3组(均P<0.01);A组动物1只死亡,余9只中6只11髋发生股骨头坏死,B组中有1只2髋在用药8周后发生了股骨头坏死,C组与D组均无股骨头坏死发生。结论:辛伐他汀和阿仑磷酸钠联合使用,能预防激素性股骨头坏死的发生、发展。Objective To explore the preventive effect of simvastatin combined alendronate on steroid-induced osteonecrosis of femoral head(ONFH) in rabbits. Methods Forty Japanese rabbits were randomly divided into 4 groups. In group A, 10 rabbits were subjected to intramuscular injection of dexamethasone 2.5 mg/kg daily;in group B, 10 rabbits were subjected to intramuscular injection of dexamethasune 2.5 mg/kg and administration of simvastatin 20 mg daily; In group C, 10 rabbits were subjected to intramuscular injection of dexamethasone 2.5 mg/kg, administration of simvastatin 20 mg and alendronate 10 mg daily;In group D, 10 rabbits were subjected to intramuscular injection of sodium chloride as control. The contents of serum cholesterol and triglyeeride were determined at 0,4,6,8 weeks after administration respectively, then the animals were sacrificed andradiographic and histological examination was performed. Results The contents of serum cholesterol and triglyceride in group A were significant higher than the other groups (P 〈 0.01 ). Femoral head necrosis were found in 6 rabbits in group A and one rabbit in group B,but no osteonecrosis were found in group C and D. Conclusion Simvastatin combined alendronate could effectively prevent the development of ONFH.
