复方丹参滴丸治疗对糖尿病合并冠心病患者认知功能的影响

摘　　要：目的:探讨复方丹参滴丸治疗对糖尿病(DM)合并冠心病(CHD)患者的认知功能和β-淀粉样蛋白(Aβ)影响。方法:糖尿病合并冠心病患者164例,随机给予复方丹参滴丸或单硝酸异山梨酯治疗,治疗前后测血糖、血脂、心肌缺血总负荷(TIB),并以简易智力状态量表(MMSE)、Blessed-Roth痴呆表和Montreal认知量表(MoCA)每4周1次,评估认知功能,以放免法测Aβ。结果:复方丹参滴丸治疗后胰岛素敏感性指数、脂代谢指标均有明显改善(P<0.01)。优于单硝酸异山梨酯治疗组,TIB改善值两组相当。复方丹参滴丸组经16周治疗Aβ水平下降(4.31±1.12)pg.mL-1,单硝酸异山梨酯治疗无此反应。复方丹参滴丸治疗16周,MMSE评分上升(1.88±0.31)分,Blessed-Roth痴呆量表评分下降(0.8±0.1)分,MOCA评分上升(3.6±0.6)分,治疗前后对比有显著性意义(P<0.05)。结论:复方丹参滴丸治疗能明显改善患者认知功能,其中机制之一是复方丹参滴丸能干预Aβ形成。Objective：To investigate the effect of Fufang Danshen Diwan （FD） on the cognitive function and β-amyloid protein （Aβ） in diabetic patients complicated with coronary heart disease （CHD）. Methods：A total of 164 diabetic patients complicated with CHD were randomly divided into 2 groups, and treated with either FD or isosorbide mononitrate （ISMN）. Before and after the treatments, blood sugar, blood lipids and total ischemia burden （TIB） were measured; the cognitive function was evaluated every 4 weeks according to the Mini-Mental State Examimation （MMSE） and Blessed-Roth Dementia Scale （B-R） and Montreal Cognitive Assessment （MoCA） ; Aβ level was measured by radioimmunoassay. Results： After treatment with FD, the insulin sensitive index and lipid metabolism index were remarkably improved （P 〈 0.01 ） as compared to ISMN group. The improvement of TIB was equal in the two groups. After 16 weeks＇treatment, the Aβ level declined by 4.31 ± 1. 12 pg/ml in FD group, but did not change in ISMN group. After 16 weeks＇FD treatment, MMSE increased by 1.88 ±0. 31 , B-R decreased by 0.8 ± 0. 1 , and MOCA increased by 3.6 ± 0. 6; there were significant differences （P 〈 0.05） before and after the treatment. Conclusion ： Fufang Danshen Diwan obviously improves the cognitive function of diabetic patients complicated with coronary heart disease; this may be due to reduction in Aβ formation.
