摘 要:目的:观察关节止痛胶囊对膝骨性关节炎血清、关节液自由基水平的影响,并探讨其作用机理及膝骨性关节炎的发病机制。方法:将符合纳入观察标准的膝骨性关节炎的患者540例随机分为关节止痛胶囊治疗组180例、壮骨关节丸对照1组180例和维骨力对照2组180例。4周为1疗程,服药8周(2个疗程)后观察疗效。测定3组治疗前后血清、关节液超氧化物歧化酶(SOD)、一氧化氮(NO)含量变化,评价治疗疗效。结果:治疗后3组患者的血清、SOD、NO含量降低,并且关节止痛胶囊治疗组作用较两对照组明显。治疗组总有效率为83.89%,显效率为50.56%。结论:关节局部氧自由基、NO是引起膝骨性关节炎的重要介质;软骨细胞凋亡过盛可能是关节软骨退变的重要原因之一。关节止痛胶囊通过提高血清、关节液SOD含量,降低其NO含量,减少软骨细胞凋亡,有效改善了膝骨性关节炎患者的临床症状,恢复膝关节功能。Objective:To observe the effect of Guanjiezhitong capsule on free radical levels in serum and articular fluid of knee osteoarthritis (KOA), and explore the possible mechanism. Methods: All 540 cases were randomly divided into 3 groups, treatment group, control group 1 and control group 2, with 180 cases in each group. Guanjiezhitong Capsule was used in treatment group, Zhuangguguanjie Pill used in control group 1, and Glucosamine Sulfate Capsule in control group 2, each treatment lasted 8 weeks. The contents of NO and SOD in the serum as well as in the articular fluid were measured before and after the treatment. Results:The content of NO in the serum and articular fluid remarkably decreased while SOD increased in all groups. The total effective rate in treatment group was 83.89%, and the marked improvement rate was 50. 56%, which showed significantly better effectiveness in the treatment group than the other 2 groups. Conclusion:Local free radicals and NO may play important roles in the development of KOA, and superfluous apoptosis of cartilage may be one of the pathways involved in the mechanism of articular cartilage degeneration. By increasing the content of SOD and decreasing that of NO in the articular fluid, Guanjiezhitong Capsule thus reduces the apoptosis of cartilage cells and effectively improves the symptoms of KOA as well as promotes the knee joint recovery.
