芪地肾膜汤联合激素治疗肾病综合征临床观察

摘　　要：目的观察芪地肾膜汤联合激素治疗肾病综合征的临床疗效。方法将69例肾病综合征患者随机分为治疗组35例和对照组34例。两组均常规给予糖皮质激素,对照组同时予雷公藤多苷片,治疗组予芪地肾膜汤。两组均以4周为1个疗程,2个疗程后统计疗效;并观察治疗后24h尿蛋白含量、血总蛋白、白蛋白、血脂、血液流变学指标、尿素氮、血肌酐、凝血酶原时间等的变化情况。结果治疗组总有效率为88.57%,对照组为64.71%;组间疗效比较,差异有统计学意义(P<0.05)。治疗组和对照组治疗后24h尿蛋白定量、尿素氮、血肌酐以及血液流变学指标、血脂、血白蛋白、总蛋白均明显改善(P<0.05,P<0.01);组间治疗后比较,除24h尿蛋白定量和尿素氮外,其余指标差异均有统计学意义(P<0.05)。结论芪地肾膜汤联合激素可以减轻肾病综合征患者临床症状,减少24h尿蛋白含量,提高白蛋白水平,降低尿素氮、血肌酐水平,降低血液黏稠度,改善脂代谢。Objective To observe the clinical efficacy of ＂Qidi Shenmo Decoction＂ and glucocorticoid in treating nephrotic syndrome. Methods Sixty-nine subjects were randomly divided into two groups： control group in which 34 cases were treated with glucocorticoid and tripterygium glycosides, and treatment group in which 35 cases were treated with glucocorticoid and ＂Qidi Shenmo Decoction＂. After eight weeks, the 24h urine protein, TP, Alb, TC, TG, ηb, ηp, Fig, HCT, BUN, Scr and APTT were observed. Results The total effective rate was 88.57% in treatment group and 64.71% in control group, with a significant difference between two groups（ P 〈0.05 ）; the 24h urine protein, BUN, Scr, blood rheolngy, blood lipid, TP and Alb were decreased after treatments in both groups （P 〈0.05, P 〈 0.01 ） ; there were significant differences between two groups in Scr, blood rheology, blood lipid, TP and Alb（P 〈0.05, P 〈0.01 ）. Conclusion ＂Qidi Shenmo Decoction＂ can improve clinical symptoms, reduce urine protein, increase Alb, decrease BUN, Scr and blood viscosity, and improve metabolism to treat nephrotie syndrome.
