腹腔镜手术治疗卵巢卵泡膜细胞瘤23例临床分析

摘　　要：目的探讨腹腔镜手术治疗卵巢卵泡膜细胞瘤的安全性及可行性。方法回顾性分析我院2001年4月至2008年6月间接受腹腔镜手术治疗的23例卵巢卵泡膜细胞瘤患者的临床资料。结果23例患者平均年龄44.2岁,其中10例行腹腔镜下患侧附件切除术,5例行腹腔镜下卵巢肿瘤切除术,3例行腹腔镜下全子宫及双侧附件切除术,2例行腹腔镜协助阴式全子宫及双侧附件切除术,2例行腹腔镜下次全子宫及患侧附件切除术,1例子宫已切除者行腹腔镜下双侧附件切除术,手术均顺利完成,平均手术时间(83.0±12.9)min,术中出血20～100mL,中位出血量45mL;LTH及LAVH术后留置尿管48h,其余术后6h拔尿管;术后平均住院时间(4.5±0.9)d,无一例出现手术并发症。术中冰冻病理检查及术后常规石蜡病理学检查证实均为良性卵巢卵泡膜细胞瘤。术后随访5～84个月,未发现复发。结论腹腔镜手术治疗卵巢卵泡膜细胞瘤是安全、可行的,术中应常规行冰冻病理学检查。Objectives To detect the safety and feasibility of laparoscopic surgery in treatment of ovarian theea cell tumor.Methods A retrospectively analysis was carried out on 23 cases with ovarian theca cell tumor and treated by laparoscopie surgery from April 2001 to June 2008.Results Among 23 patients,the mean age is 44.2 years. Ten patients underwent laparoscopic unilateral adnexectomy,and 5 for resection of ovarian tumor, 3 for laparoscopic total hysterectomy and bilateral adnexectomy,2 for laparoscepiclly assisted vaginal hysterectomy and bilateral adnexectomy, 2 for laparoscopic subtotal hysterectomy and unilateral adnexectomy, l for laparoscopic bilateral adnexectomy who had hysterectomy before. All the operations were successfully done without any intro-- or post-- operative complication. The mean operation time is （83.0±12.9）min,and the mid--term bleeding during operation is 45ml.The resume time of bladder after LTH or LAVH is 48 hours and the time of other operations is 6 hours. The mean hospitalized time after operation is 4.5±0.9 days.All cases were diagnosed as benign ovarian theca cell tumor by intr--operative frozen sections and post--operative routine pathologic examination. No recurrence was found during the follow--up （range from 5 to 84 months）. Conclusion Laparoscopic surgery is safe and feasible for treatment of ovarian theca cell tumor. And intr--operative frozen sections for pathologic examination should be done as routine.
