摘 要:目的了解踝肱指数(ABI)对诊断糖尿病下肢血管病变(LEAD)的意义及其相关因素分析。方法对106例2型糖尿病患者测定ABI,根据ABI分为四组:正常组(1.0≤ABI≤1.3)、轻度缺血组(0.8≤ABI<1.0)、中度缺血组(0.5≤ABI<0.8)、重度缺血组(ABI<0.5),并对ABI与病程、糖化血红蛋白(HbA1c)、低密度脂蛋白(LDL-C)、血压进行相关分析。结果106例2型糖尿病患者中ABI<1.0符合LEAD诊断者36例,相关因素分析发现LEAD患者的病程、HbA1c、LDL-C、收缩压(SBP)均明显大于正常组(P<0.05),与ABI呈负相关。结论ABI值低者糖尿病病程长、糖化血红蛋白高、低密度脂蛋白高、血压高,提示病程、LDL-C、HbA1c、SBP是下肢血管病变的危险因素。Objective To investigate the clinical significance of Ankle - brachial index (ABI) for the diagnosis of diabetic lower - extremity arterial disease (LEAD) ,to analyse the relationship of ABI with cardiovascular risk factors. Methods ABI was examined in the 106 patients with T2DM. According to ABI, all subjects were divided into four groups as normal group ( 1.0 ≤ ABI ≤1.3 ) , mild - ischemia group (0.8 ≤ ABI 〈 1.0), mid - ischemia group (0.5 ~〈 ABI 〈 0.8 ) and severe - ischemia group ( ABI 〈 0.5 ). At the same time , course of disease, glycosylated hemoglobin (HbAlc), low density lipoprotein cholesterol (LDL- C ) and systolic blood pressure (SBP) were also detected. Results Among the 106 patients with T2DM , 36 with abnormal ABI 〈 1.0 , Correlation analysis discovered that course of disease , HbAlc, LDL -C and SBP among patients with LEAD are more higher than those among normal group (P 〈 0.05 ), and negative correlated with ABI. Conclusion The patients of lower ABI have longer course of disease, high level of HbAlc and LDL - C, hypertension. Therefore course of disease, HbAlc,LDL- C and SBP might be LEAD's risk factors.
