摘  要:目的探讨鹿瓜多肽注射液对肢体缺血再灌注损伤保护作用的机制和时效及量效关系。方法选择合适病人如断肢再植、皮瓣移植、四肢大血管损伤、骨筋膜室综合征等或四肢手术须用止血带1~1.5h的男性患者45例,随机分为A、B、C、D和E组(空白对照组)五组,A、B组在手术前30min、术后30min分别以鹿瓜多肽注射液4mg加入5%糖盐水(GNS)静脉滴注,C、D组在手术前30min、术后30min分别以鹿瓜多肽注射液12mg加入5%GNS静脉滴注,E组单纯静脉滴入5%GNS,三组分别在术前1h、术后6h抽取静脉血,测定MDA、SOD、CPK、GOT和LDH含量。结果各治疗组中SOD明显增高,MDA、CPK、GOT则明显降低。结论鹿瓜多肽注射液对肢体缺血再灌注损伤具有保护作用,但术前治疗效果更佳。Objective To study the mechanism, time-effect relationship, dose-effect relationship about the protection functions for the reinfusion injury of ischemic limbs from Cervus and Cucumis Polypeptide Injections. Methods Divide 45 male patients into 5 groups as A,B,C,D,E (Blank control group) in random.,who had limb replantations, flap transplantations, limb vascular injuries, bone compartment syndromes or limb surgeries with tourniquet for 1.0~1.5 hours. Group A and Group B were dripped intravenously with Cervus and Cucumis Polypeptide Injections. with 4 mg and 5% glucose in normal saline 30 minutes before and after the operations respectively. Group C and Group D were dripped intravenously with Cervus and Cucumis Polypeptide Injections. with 12 mg and 5% glucose in normal saline 30 minutes before and after the operations respectively. Group E was dripped intravenously with 5% glucose in normal saline only. Five groups patients were collected bloody samples from veins 1 hour before operations and 6 hour after operations. Then det6rmine the concentrations of the MDA, SOD,CPK, GOT in the bloody samples.Results SOD increased markedablely; MDA, CPK, GOT decreased markedablely in the therapied groups.Conclusion Cervus and Cucumis Polypeptide Injections can surely protect the reinfusion injury of the ischemic limbs, with better effects admitted pre-operations.
