血栓前体蛋白(TpP)用于心肌梗塞诊断的研究

摘　　要：目的:研究血浆中血栓前体蛋白(TpP)的检测在心肌梗塞诊断中的意义。方法:利用酶联免疫分析法测定30例健康对照组和30例急性心肌梗死(AMI)患者血浆中前体蛋白的水平。结果:急性心肌梗死组与对照组男性、女性分别相比,急性心肌梗死组血浆TpP水平均显著高于对照组(P<0.01),AMI组TpP和D-Dimer水平均高于对照组(P<0.05);AMI组cTn I、CK-MB和TpP的阳性率均显著高于对照组(P<0.01);TpP在心肌损伤的4小时内就开始升高,4～8小时内达到高峰后有下降趋势,而cTn Ⅰ在心肌损伤的4小时内未见升高,4～12小时内维持在较高水平,TpP阳性率先升高后降低,CK-MB和cTn I阳性率一直升高;AMI组中,小于2小时,TpP的敏感性、阴性预测值和准确性均显著高于CK-MB和cTn I(P<0.01,P<0.05);3～4小时,TpP的敏感性、阴性预测值和准确性均显著高于CK-MB和cTn I(P<0.01),TpP异性显著低于cTn I(P<0.01)。结论:急性心肌梗死患者血浆TpP水平显著高于正常人,血浆TpP水平动态研究发现,其可作为诊断心肌梗塞的一项敏感指标,对心肌梗塞的早期溶栓治疗有指导作用。Objective：To study the significance of Thrombus Precursor Protein（TpP） in the Diagnosis of Miocardial infarction.Methods：To use TpP ELISA determine the level of TpP in 30 cases healthy people and 30 cases Acute Miocardial infarction.Results：The levels of TpP in blood plasma of .femal and mail AMI were higher than the healthy people,the differences had the significanee（P〈0.01）.The TpP and D-Dimer in AMI were all higher than the beaithy people,and the differences had the significance（P〈0.05）.The positive rate of cTnI, CK-MB ang TpP in AMI were higher than the healthy people.and the differences had the significance （P〈0.01）.The TpP rose in 4 hours after the scathe of cardiac muscle,and it reached the apex from 4 to 8 hours,then it declined.Meanwhile,CK-MB and cTn I rose continually.The sensitivity and veracity of TpP were higher than that of CK-MB and cTnI,and the differences had the significance （P〈0.05）.Conclusions：The TpP in AMI was higher than the healthy people,and the dynamic study showed that the TpP can be used as a sensitive index in the Diagnosis of Miocardial Infarction,and it was significant in the early treatment of Miocardial Infarction.
