彩色多普勒超声对慢性肾病肾血流动力学观察

摘　　要：目的采用彩色多普勒超声观察慢性肾脏病不同时期,肾内各分级动脉血流动力学改变及其临床价值。方法选择慢性肾脏病患者50例,根据肾脏病理、血肌酐、尿素氮等指标进行分组,分别分为:肾病Ⅰ组、肾病Ⅱ组、肾病Ⅲ组,与正常对照组50例进行比较,应用彩色多普勒超声波诊断仪,观察肾脏体积大小,皮髓交界是否清晰,皮质回声强度及皮质厚度。测量双肾大小并观察其内部回声,彩色多普勒显示肾内各分级动脉血流充盈情况,即段动脉、叶间动脉,测量各血流参数,包括收缩期峰值流速(Vs)、舒张末期流速(Vd)、阻力指数(RI),并相互进行对照。结果慢性肾脏病Ⅱ组及Ⅲ组肾段动脉及叶间动脉收缩期最大血流速度(Vs)、舒张期最低血流速度(Vd)及阻力指数(RI)较对照组比较,其差异显著,肾脏病理损害越严重,肾功能损害越严重,上述指标越明显。结论彩色多普勒检测能无创伤监测慢性肾脏病肾血流动力学的变化,为疾病的诊断、病情的监测与治疗效果提供重要的临床参数。Objective To observe the changes of the renal hemodynamic indexes at different phases of chronic nephritis（CN） by color Doppler ultrasonography and study its clinical value. Methods Fifty cases with CN were divided into group Ⅰ , group Ⅱand group Ⅲ according to the indexes of nephridial pathology, creatinine and BUN （ blood urea nitrogen）. 50 healthy subjects were as control. Color Doppler ultrasonography was adopted to observe the volume of the kidney,whether the border of cortex and medulla is clear,and the intensity of the cortex echo and the thickness of it. By measuring the volume of the kidney and observing the echo, color Doppler showed the segment artery and fullness condition of interlobar artery. The indexes including the maxim blood flow rate in systolic phase（Vs） ,the minimum blood flow rate in diastolic phase（Vd） and resistant index（RI） were measured and compared. Results Compared with the control group, the maxim blood flow rate in systolic phase and the minimum blood flow rate in diastolic phase and the resistant index of group Ⅰ and Ⅱchanged obviously. The worse the damage of kidney pathology and nephridial function got aggravated, the more obvious these indexes were. Conclusion Color Doppler ultrasonography can measure the changes of the renal hemodynamic indexes of CN without hurt,which provides important data for the diagnosis, controlling and treatment of diseases.
