老年心力衰竭患者血浆B型利钠肽与血尿酸关系研究

摘　　要：目的探讨老年心衰患者血浆B型利钠肽(BNP)与血清尿酸(UA)关系。方法选择44例老年器质性心脏病患者,按纽约心脏病协会(NYHA)心功能分级分为Ⅱ级14例,Ⅲ级19例,Ⅳ级11例,分别测定血浆BNP及血清UA,比较不同心功能级别患者的血浆BNP水平与血清UA水平。结果老年心衰患者血浆BNP及血清UA明显高于正常对照组[(697.04±535.49)pg/mlvs(81.15±21.24)pg/ml,(462.73±100.68)μmol/Lvs(198.47±44.30)μmol/L]。心功能Ⅱ、Ⅲ、Ⅳ级各组患者血浆BNP水平分别为(316.11±61.09)pg/ml、(635.95±280.2)pg/ml、(1287.36±702.36)pg/ml。血清UA水平分别为(377.00±51.93)μmol/L、(473.47±98.34)μmol/L、(553.27±56.08)μmol/L。各组血浆BNP与血清UA水平两两比较,差异有统计学意义(P<0.01,P<0.05)。结论血浆BNP水平、血清UA水平与心功能分级有相关性。Objective To explore the relationship between plasma B-type natriuretic peptide（BNP） and serum uric acid（UA） level in elderly patients with heart failure. Methods 44 old patients with heart disease were divided into Ⅱ、Ⅲ、Ⅳ groups by NYHA cardiac function classification, and the level of plasma BNP and serum UA determined and compared. Results The levels of plasma BNP and serum UA in elderly patients with heart failure were significantly higher than that of the control group [ （697.04 ±535.49 ） pg/ml vs （ 81.15 ±21.24 ） pg/ml, （462.73±100.68 ） μmol/L vs （ 198.47 ±44.3 ） μmol/L ]. The levels of plasma BNP in Ⅱ Ⅳand Ⅳgroups were （ 316.11 ±61.09 ） pg/ml, （ 635.95 ± 280.2 ） pg/ml and （ 1 287.36 ±702.36 ） pg/ ml respectively. The serum UA levels in the three groups were（377.00 ±51.93） μmoL/L, （473.47 ±98.34） μmol/L and （ 553.27 ± 56.08 ）μmoL/L respectively. The differences of levels of BNP and UA between each group were significantly （ P 〈0.05 ）. Conclusion The levels of plasma BNP and serum UA were positively correlated with cardiac function classification.
