两种微创方法治疗子宫肌瘤的费用-效果分析

摘　　要：目的比较子宫动脉栓塞(uterine artery embolization,UAE)与腹腔镜子宫肌瘤剔除术(laparoscopic myomecto-my,LM)治疗子宫肌瘤的费用-效果关系。方法2003年1月-2005年12月在广东药学院附属第一医院等采用微创治疗的157例子宫肌瘤患者,其中LM68例(腹腔镜组),UAE89例(介入组),比较两组患者的费用、疗效、平均住院日,计算两组的费用-效果比,并对术后远期效果及复发情况进行随访。结果157例子宫肌瘤患者实施微创治疗全部获得成功,两组疗效对比差异无统计学意义(P>0.05)。介入组与腹腔镜组例均费用分别为(6091±1682)元和(8216±1953)元,平均住院日分别为(4.1±1.2)d和(6.8±1.3)d,费用-效果比分别为60.9和82.2,经统计学检验差异有统计学意义(P<0.05或0.01)。随访至术后3年以上,两组长期效果及复发率差异无统计学意义(P>0.05)。结论相对于LM,UAE是一种费用-效果比更优的治疗方法。Objective To compare the cost-effectiveness of uterine artery embofization （UAE） with that of laparoscopic myomectomy（LM） in treating hysteromyoma. Methods The clinical data, including cost, curative effect and mean clays in hospital, of 157 cases with hysteromyoma（68 treated by LM and 89 by UAE） from Jan. 2003 to Dec. 2005 were analyzed retrospectively. The cost-effectiveness was evaluated. The postoperative long term effects and recurrence were investigated by 3 years of follow up. Results The micro-invasion operations for 157 patients were successful. There was no statistic difference in therapeutic effect between the two groups（P 〉0.05）. UAE group had significantly lower hospitalization cost（6 091 ± 1 682 yuan vs 8 216 ± 1 953 yuan respectively）, shorter hospital stay [ （ 4. 1 ± 1.2 ） d vs （ 6.8 ± 1.3 ） d respectively ] and lower cost-effectiveness value （60.9 vs 80.2 respectively） as compared with the LM group（P 〈 0.05 or 0.01 respectively）. The follow up showed that there was no statistic difference in long term curative effects and recurrence. Conclusion Compared with LM, UAE is the better method according to the results of cost-effectiveness analysis.
