PPH与传统手术方法治疗环状混合痔疗效对比分析

摘　　要：目的探讨PPH(procedure for prolapse and hemorrhoids)与传统(分段外剥内扎)手术方法治疗环状混合痔的临床效果。方法选取2008年1月～2009年1月间在我院住院治疗的环状混合痔患者108例,随机分为PPH组和传统手术组,观察两组患者术中、术后一般情况及并发症发生情况。结果PPH组手术时间、术中出血量均明显少于传统手术组,两者比较差异具有统计学意义(P<0.05)。PPH组住院时间、恢复工作时间明显短于传统手术组,两者比较差异具有统计学意义(P<0.05)。PPH组术后疼痛指数、尿潴留、肛门肿胀、便中带血及术后切口渗血发生率明显少于传统手术组,两者比较差异具有统计学意义(P<0.05)。PPH组术后复发率以及手术有效率与传统手术组比较差异无统计学意义(P>0.05)。结论PPH治疗环状混合痔具有传统方法无法比拟的优点,值得应用。Objective To invesgate the curative effect of procedure for prolapse and hemorrhoids and traditional operation（ex-strip and intra-wrapping in section） in treating circular mixed hemorrhoid. Methods 108 cases with circular mixed hemorrhoids were divieded into the PPH group and the traditon group, the general state of health in and after operation, complications were observed. Results The time of operation and hemorrhage in operation in the PPH group were fewer than those in the tradition group, and there was significant difference between two groups（P 〈 0.05）. The length of stay and returning to work in the PPH group were fewer than those in the tradition group, and there was significant difference between two groups（P 〈 0.05）. The rate of pain index after operation, urine retention, anus swelling, bleed attaeh to sedes and capillary hemorrhage in nick after operation in the PPH group were fewer than those in the tradition group, and there was significant difference between two groups（P 〈 0.05）. There was no significant difference between two groups in relapse rate relapse after operation and effective power （P 〉 0.05）. Conclusion PPH in treating circular mixed hemorrhoid is superior to traditional operation, which is worthy of clinical application.
