前列腺疾病患者血清硫酸脱氢表雄酮的检测及其临床意义

摘　　要：目的：探讨前列腺疾病患者血清硫酸脱氢表雄酮（dehydroepiandrosterone sulfate,DHE-AS）水平的变化及其在其发展过程中的临床意义。方法：用化学发光法测定95例前列腺疾病患者和30例正常对照组血清DHEAS,以及总前列腺特异性抗原（total prostate specific antigen,TPSA）及总睾酮（total teslen,TT）水平。结果：前列腺增生组和前列腺癌组患者血清中DHEAS和TT水平均明显低于对照组,差异具有统计学意义（P〈0.001）;前列腺癌组患者血清中DHEAS和TT的水平又明显低于前列腺增生组,差异具有统计学意义（P〈0.05~0.001）;而前列腺增生组和前列腺癌组患者血清中TPSA水平与正常对照组比较则明显增高,差异具有统计学意义（P〈0.05~0.001）。前列腺增生组和前列腺癌组患者血清中DHEAS水平与TPSA的水平呈负相关（r=-0.619,P〈0.05）,与TT水平呈正相关（r=0.571,P〈0.05）。结论：DHEAS在前列腺癌的发病中可能起着一定的作用,但其在前列腺癌发病中的确切作用机制及是否为前列腺癌发病的独立因素抑或通过其它危险因素引起作用尚需进一步研究。Objective： To study the level change and clinical significance of the serum dehydroepiandrosterone sulfate （ DHEAS ） in patients with disease of prostate. Method：Using chemiluminescence technique to measure the serum levels of DHEAS, total prostate specific antigen （TPSA） and total teslen （TT） in 95 patients with disease of prostate and 30 normal controls. Result：The serum levels of DHEAS and TT were significantly lower in patients with disease of prostate than that of the control group（ P 〈0. 001 ）. The serum levels of DHEAS and TT were significantly lower in patients with prostate cancer than that of hyperplasia of prostate gland patients （ P 〈 0.05 - 0.001 ）. The serum level of TPSA was significantly higher in patients with disease of prostate and prostate cancer than that of the control group{ P 〈 0.05- 0.001 ）. The serum level of DHEAS was negatively correlated with the serum level TPSA in patients with disease of prostate and prostate cancer （ r = - 0.619, P 〈 0.05 ） , and was positively correlated with TF （ r = 0. 571, P 〈 0.05 ）. Conclusion： DHEAS might play a role in the morbility of prostate cancer, but it plays a role on the exact mechanism of prostate cancer and the incidence independent of other factors or a danger factors role needs further study.
