摘 要:目的:了解心肌酶谱对急性心肌梗死的诊断价值。方法:应用受试者特征(ROC)曲线对129例急诊怀疑为急性心肌梗死的发病24h内就诊患者进行评价。结果:(1)急性心肌梗死组患者心肌酶谱各指标的平均水平均高于非急性心肌梗死组患者;(2)各指标的ROC曲线下面积以肌酸激酶同工酶CKNA为最大(A=0.873),其余各项指标的面积介于0.712至0.758之间。结论:除CKNA具有较高的诊断价值外,其余各项指标的诊断价值不大。CKNA似以160u作为诊断参考值最佳,此时灵敏度为85.1%,特异度为82.3%。ABSTRACT Objective: To understand the diagnostic effects of myocardial enzymes towards acute myocardial infarction (AMI). Methods: By using the receiver operating characteristic curve, this essay evaluates the diagnostic effects of myocardial enzymes to 129 cases who were suspected of AMI and attacked within 24 hours. Results: (1) The average levels of all myocardial enzymes in AMI group are higher than those in nonAMI group. (2) CKNA has the highest undercurve area (A=0.873), and the areas of other items range from 0.712 to 0.758. Conclusion: Only CKNA shows a satisfied diagnostic effect, and it seems that the best cutoff point may be 160 unit which generates a sensitivity of 85.1% and a specificity of 82.3%.
