摘 要:比较容量控制反比通气(VC-IRV,简称IRV)和气道压力释放通气(APRV)模式对健康和急性肺损伤(ALI)犬心肺功能的影响。方法对健康和ALI模型(犬),分别用IRV和APRV模式进行通气,观察通气期间血流动力学、血气、呼吸生理的变化。结果对于健康犬在气道峰压相似的情况下,APRV模式时平均气道压和肺动脉压显著高于IRV时。对血流动力学、血气和呼吸力学的影响两者差别不显著。APRV模式下氧运输量和PaO2/FiO2值较IRV对显著高。对于ALI犬APRV时的气道峰压显著低于IRV时,平均气道压两者相似,PaO2/FiO2则是APRV时显著高于IRV时。结论APRV与IRV相比,前者氧合功能显著优于后者,且气道峰压较低,更适用于ALI者。おURPOSE In order to compare the effects of cardiac-pulmonary function in health and acute lung injury (ALI) dogs by inverse ration ventilation (IRV) and airwary pressure release ventilation (APRV) ventilatory modes.METHODS Two different ventilatory modes, volume control IRV and APRV were given to ten health and ALI dogs. During ventilation, the parameters of hemodynamics, blood gas and respiratory physiological indices were recorded.RESULTS For the health dogs, when the peak airwary pressure were simillar, the mean ariwary pressure of APRV was notably higher than that of IRV, while their effects on hemodynamics and blood gas were simillar. For ALI dogs, the peak airwary pressure of APRV was significantly lower and PaO2/FiO2 significantly higher than that of IRV, while the mean airwary pressure of two modes were simillar.CONCLUSIONS The oxygenation of APRV is better than that of IRV, and with lower peak airway pressure, APRV is more fit for ALI patients.
