摘  要:目的:观察负压疗法对糖尿病伴有肢体血管病变患者的肢体血管弹性、阻力和血流量的影响。方法:将87例患者分为两组,负压疗法组47例,药物治疗组40例,两组在性别和年龄上差异无显著性。负压疗法采用筒形负压舱,舱内压力上肢为-10.6kPa(1kPa=7.5mmHg),下肢为-13.3kPa,治疗每日1次,15分钟,10~15次为1疗程。分段检测肢体的血管弹性、阻力和血流量。结果:负压疗法组治疗后血管弹性和阻力均得到改善,尤以踝趾段和小腿段血流量均有增加,负压疗法组显著优于药物组(χ2分别为56.549,24.956,P<0.01)。两组中有47例治疗后血流量未改善,其中37例经肢体多普勒超声图证实有严重的血管病变。结论:负压疗法对糖尿病患者并发早、中期肢体血管病的血管功能有一定的改善,是一种安全、方便。Objective:In order to observe the effect of negative pressure on vascular elasticity,resistance and bloodflow volume in diabetic patients with peripheral angiopathy.Methods:87 diabetic cases with peripheral angiopathy were randomly divided into two groups:the treatment group(47 cases)were treated with negative pressure therapy.The tubeshaped pressure chamber was used.The pressure in the chamber for arms and legs was -10.6kPa and -13.5kPa respectively,once a day,15min/time for 10~15 times.Another 40 cases as a control group were only managed by the drugs.After the treatment,the vascular elasticity,resistance and bloodflow volume in the limbs in both groups were segmentally examined.Results:The vascular elasticity and resistance after negative pressure therapy were improved,particularly in the ankletoe and wristfinger segments(P<0.05).The bloodflow volume of ankletoe and leg segments was increased in both groups,but the blood volume of treatment group was superior to that of control group.In addition,47 cases were unchanged in blood volume in both groups.Among them,37 cases had severe angiopathy proved by Doppler ultrasonoscope.Conclusion:It is shown that the negative pressure therapy is a safe,convenient and noninvasive therapy for the early or middle diabetic patients with peripheral angiopathy.
