摘  要:目的通过潮气呼吸肺功能对毛细支气管炎和肺炎患儿进行检测,了解其在临床中的应用,并探讨其影响因素。方法毛细支气管炎患儿30例和支气管肺炎患儿30例纳入本次研究。患儿镇静后进行潮气呼吸肺功能检测,主要测定参数为每公斤体重潮气量(VT)、达峰时间比(TPTEF/TE)、达峰容积比(VPTEF/VE)。结果毛细支气管炎组与肺炎组每千克体重潮气量(VT)差异无统计学意义(P>0.05)。毛细支气管炎组达峰时间比(TPTEF/TE)、达峰容积比(VPTEF/VE)低于肺炎组,差异具有统计学意义(P<0.05)。结论潮气呼吸肺功能测定是一项很好的检测婴幼儿肺功能的技术,可评价喘息患儿气道阻塞程度,但在测定过程中,应注意对可能影响肺功能测定的因素加以控制,否则将影响测定结果。Objective To study the clinical application and its impact factors of tidal flow - volume measurement in infants with bronchiolitis and pneumonia. Methods Thirty infants with bronchiolitis and thirty infants with pneumonia were studied. Tidal breathing flow - volume loops were obtained after the patients were sedated. The tidal volume per kilogram (VT/kg) , the fraction of tidal volume exhaled at peak tidal expiratory flow (PTEF) to total tidal volume (VPTEF/VE) , and the fraction of exhaled time at PTEF to total expiratory time (TPTEF/TE) were measured. Results VT/kg was not significant different between the bronchiolitis group and the pneumonia group. VPTEF/VE and TPTEF/TE were lower in the bronchiolitis group than those in the pneumonia group. Conclusion The measurement of tidal breathing flow - volume loops is a good technology, which would be helpful to evaluate airway obstruction in wheezing in- fants. The impact factors should be controlled during the measurement. Otherwise, the results would be affected.
