放置宫内节育器或行输卵管银夹者在1.5T MRI检查的安全探讨

摘　　要：目的:探讨育龄妇女放置宫内节育器或行输卵管银夹做1.5TMRI检查的安全性。材料与方法:已放置了宫内节育器或行输卵管银夹的妇女共180例。其中放置宫内节育器的患者130例,最短时间为6个月,最长时间为15年,平均5.6年;输卵管银夹的患者50例,最短时间为3个月,最长时间为8年,平均为4.3年。全部患者在1.5T超导MRI上检查,检查部位为头、颈、脊椎、腹部,避开盆腔,检查时间为10～120min,平均36.8min。检查前当天、检查后当天或第二天,宫内节育器的患者做B超检查,监测宫内节育器在宫腔内的位置;输卵管银夹的患者做腹部透视,排除银夹移位、脱落及不适进行记录。对检查图像进行质量评估;将检查前后的数据进行统计学分析。结果:图像优良等级达100%;MRI检查前后宫内节育器、输卵管银夹的位置无显著性差(P>0.05),未发生任何不良反应。结论:育龄妇女在放置宫内节育器或输卵管银夹结扎术后,只要避开盆腔,在1.5TMRI检查是安全的,不需要取出节育器或输卵管银夹,但通常要求宫内节育环放置6个月以上,银夹手术3个月以上。Purpose： To study the safety of high - field MRI in women with Intrauterine device （IUD） or Tubal sterilization operation with silver. Materials and Methods： 180 eases with Intrauterine deviee（IUD）or Tubal sterilization operation with silver. 130 eases with Intrauterine device（ IUD）, the Intrauterine device（IUD） from implantaion to MR/examination varied from 6 months to 15 years with an average 5.6 years; 50 cases with Tubal sterilization operation with silver, the silver from implantaion to MR/examination varied from 3 months to 8 years with an average 4. 3 years. All women received MR/examination at a 1.5T super- eonduetive MR/. Cheek site of head, neck, spine, abdomen in addition to outside the pelvie, examination duration time varied from 10 to 120 minutes with an average of 36. 8 minutes. Before, after or 1 day after the examination, the Intrauterine device（IUD） were checked with transvagiual ultrasonic scan, the tubal sterilization operation with silver with X - ray and body eomplaiuts （if any ）were recorded. Image quality was also evaluated; statistical analysis was performed. Results： 100% of images were with excellent quality, no side effect was recorded, the site of before, after or 1 day after the examination which showed no significant difference （p 〉 0. 05 ）. Conclusion： Women with Intrauterine device （IUD）or Tubal sterilization operation with silver can receive 1.5T high field MR/examination safely to outside the pelvic, it was do not need to remove the tutrauterine device （IUD）or tubal sterilization operation with silver, but the examination can be performed in 6 months after the inserting tntrauterine device（IUD） and 3 months after the tubal sterilization operation with silver.
