早期护理干预对脑卒中伴吞咽障碍患者预后的影响

摘　　要：目的:探讨早期护理干预对脑卒中伴吞咽障碍患者吞咽功能的恢复以及减少吞咽障碍所引起并发症的影响。方法:将120例患者随机分为实验组和对照组各60例,对照组给予常规治疗和护理,实验组患者在给予常规治疗和护理的基础上,根据吞咽功能级别分别给予不同的饮食指导和吞咽功能训练。结果:实验组患者吞咽功能康复情况明显优于对照组,两组之间存在显著性差异(P<0.01);治疗组吸入性肺炎15例(25.0%),营养不良9例(15.0%),对照组吸入性肺炎27例(45.0%),营养不良16例(26.7%),两组比较有显著性差异(P<0.05)。结论:早期进行有效吞咽功能的康复治疗和针对性的饮食指导,不仅可减少并发症的发生,而且可提高患者的日常能力和生活质量,对脑卒中患者的最终预后产生了积极影响。Objective ： To study the impact of early nursing intervention on recovery of the patients and reducing the complications caused by dysphagia in the treatment patients with stroke associated with dysphagia. Methods： 120 patients were randomly divided into experimental group and control group （60 cases for each group）. Routine treatment and nursing care was taken in control group and divers guidance and training on diet and swallowing function were applied in accordance with the grading of swallowing function in experimental group. Results ： The rehabilitation of swallowing function of the patients in experimental group was obviously superior to that in control group （P 〈 0.01 ） ; the significant differences were found in the comparison of occurrence of aspiration pneumonia and malnutrition between the two groups （ P 〈 0.05 ）. Aspiration pneumonia occurred in 15 patients （25.0%） and malnutrition in 9 patients （ 15.0% ） in experimental group; aspiration pneumonia in 27 patients （45.0%） and malnutrition in 16 patients （26.7%）. Conclusion： The early and effective rehabilitation of swallowing function and targeted dietary guidance can not only reduce the incidence of complications, but also improve daily living ability and quality of life of the patients, which will have a positive impact on ultimate prognosis of patients with stroke.
