PVB方案加同步放疗治疗中晚期宫颈癌临床分析

摘　　要：目的:探讨PVB(博来霉素+长春新碱+顺铂)加同步放疗方案与单纯同步放疗对中晚期宫颈癌患者的疗效。方法:将1999年9月～2002年7月我院收治的中晚期宫颈癌患者104例,随机分为A组(PVB方案同步放疗组)52例,B组(单纯放疗组)52例,将2组的治疗后肿瘤消退情况、生存率、复发率及远处转移情况和不良反应进行比较。结果:治疗3月后A组和B组肿瘤消退治疗有效率分别为96.2%和71.2%,其差异有显著性意义(P<0.05);A组和B组生存率分别为84.6%和57.7%,两组比较有显著差异性(P<0.05)。A组复发率为7.7%,远处转移率为9.6%,B组复发率17.3%,远处转移率23.1%。B组复发率及远处转移率显著高于治疗组(P<0.01);A组和B组Ⅲ～Ⅳ度骨髓抑制分别为13%和10%,A组与B组比较,化疗不良反应较著,但是差异无显著性意义(P>0.05)。结论:PVB和同步放化疗是治疗中晚期宫颈癌有效的方案,与单纯同步放疗比较,PVB方案加同步放疗对中晚期宫颈癌的效果更好。OBJECTIVE：To evaluate the efficacy of the PVB program（bleomycin ＋ vincristine ＋ cisplatin） plus simultaneous radiotherapy vs.radiotherapy alone for intermediate-advanced cervical cancer.METHODS：A total of 104 patients with intermediate-advanced cervical cancer admitted to our hospital from Sept.1999 to July 2002 were randomly divided into Group A（PVB program plus synchronous radiotherapy,n=52） and Group B（radiotherapy alone,n=52）.Tumor regression,survival rate,relapse rate and distant metastasis,and adverse reactions after treatment were compared between the two groups.RESULTS：After 3-month treatment,there were significant differences between Group A and Group B in the effective rate on tumor regression（96.2% vs.71.2%,P〈0.05） and survival rate（84.6% vs.57.7%,P〈0.05）.The recurrence rate and distant metastasis rate were significantly higher in Group B than in Group A（7.7% and 9.6%,respectively in Group A versus 17.3% and 23.1%,respectively in Group B,P〈0.01）.Ⅲ～Ⅳ degree of bone marrow suppression in Group A and Group B were 13% and 10%,respectively.The adverse reactions of chemotherapy in Group A were more severe than in Group B,but the difference was not significant（P〉0.05）.CONCLUSION：PVB program plus synchronous radiochemotherapy is effective and better than radiotherapy alone for intermediate-advanced cervical cancer.
