摘  要:目的探讨维持性血液透析(MHD)患者高尿酸血症及微炎症状态与心血管疾病的关系。方法选择我院MHD患者48例,记录所有患者的心血管疾病病史、性别、年龄、血压、身高、体质量,同时透析前采血查炎症标志物C反应蛋白(CRP)及血清尿酸(UA)、白蛋白、血红蛋白、尿素、血清总胆固醇、低密度脂蛋白等。分析血清UA及CRP水平与心血管疾病的关系。结果48例患者中有心血管疾病患者29例,无心血管疾病患者19例,心血管疾病组的UA及CRP明显高于无心血管疾病组(P<0.01)。高尿酸血症伴CRP升高组的心血管疾病发生率明显高于其他组(P<0.01)。多元Logistic逐步回归分析证实:UA及CRP与发生心血管疾病相关。结论维持性血液透析患者心血管疾病与高尿酸血症及微炎症状态高度相关。Objective To identify the relationship of hyperuricemia and chronic inflammation and their cooperation in cardiovascular disease in maintenance hemodialysis patients. Methods The study included 48 MHD patients in our dialysis center. Histories of cardiovascular disease, sex, age, blood pressure, height, body weight were recorded. Serum was sampled for the determination of UA, CRP, albumin, urea, Hb, total cholesterol and lowdensity lipoorotein. UA and CRP correlation with cardiovascular disease were evaluated. Results Twenty-nine patients had cardiovascular disease. Patients with cardiovascular disease had significantly higher UA and CRP (P〈 0. 01) than patients without cardiovascular disease. Patients in hyperuricemia with elevated CRP group had higher incidence of cardiovascular disease as compared with patients in others group. Multivariate regression analysis demonstrated that UA and CRP were predictors of cardiovascular diseases. Conelusion The results showed hyperuricemia and chronic inflammation in maintenance hemodialysis patients are the risk factors leading to cardiovascular complications.
