场所与街头暗娼艾滋病知识、性行为比较和安全套使用影响因素分析

摘　　要：目的比较广东省某市场所暗娼与街头暗娼在艾滋病感染相关性行为方面的差别,并探索安全套使用的影响因素,为制定针对暗娼的安全套推广策略提供科学的依据。方法在广东省某市采用同伴推动抽样法(RDS)招募社区内场所暗娼和街头暗娼,采用统一的问卷表以面对面的方式进行定量调查,收集暗娼的艾滋病感染相关性行为等信息。结果本次研究分别招募场所暗娼和街头暗娼241和77名。街头暗娼与场所暗娼相比,文化程度低、为家庭提供经济支持的比较较高、从事商业性行为的时间较长、对商业性性行为的收费较低、商业性性伴数较多、与商业性性伴的安全套使用率较低、上一次与固定性伴的安全套使用率较低。多因素Logistic回归模型分析显示,街头暗娼、有固定性伴、以及有非商业性临时性伴是不坚持使用安全套的独立危险因素。结论街头暗娼比场所暗娼具有更高的艾滋病感染相关危险行为,应是今后艾滋病干预的重点。对暗娼干预时要考虑暗娼的经济压力和覆盖其非商业性性伴。Objective To compare the HIV infection related sexual behaviors between place - based and street - based sex workers in a prefecture in Guangdong Province and explore the influence factors associated with condom use in order to provide a scientific basis for future condom promotion programs. Methods Respondent driven sampling （ RDS） was used to recruit establishment - based and street - based sex workers in the community in a prefecture in Guangdong Province. Face to face interviews were conducted to collect information on risk behaviors using structured questionnaires. Results In total 241 establishment - based and 77 street - based sex workers were recruited, accounting for 75.3% and 24.1% respectively. The street - based sex workers had lower education level and had a higher proportion who were supporting their family. They had engaged in commercial sex for a longer period, charged a lower price for commercial sex and had more commercial sex partners. Compared with establishment - based sex worker, the street - based sex workers had a lower proportion of con- dom use with clients and with regular sex partners for the last sex act. The multivariate Logistic regression model indicated that being a street - based sex worker, having regular sex partner, and having non - regular non - paying sex partner were independent risk factors associated with inconsistent condom use. Conclusions Street - based sex workers had more risk behaviors than establishment - based sex workers and should be focused at in the future intervention programs. The poor income status of sex workers should be brought into considesation and non - business sex partners should also be included in making intervention programs.
