种植体支抗压低上切牙改善露龈笑的临床初探

摘　　要：目的应用种植体支抗压低上切牙改善露龈笑，探讨种植体支抗压低上切牙改善露龈笑的可行性及有效性。方法选择16例露龈笑伴前牙深覆骀女性患者，平均23．8岁，于上颌双侧侧切牙与尖牙间唇侧角化龈处植入微螺钉种植体，配合全口固定矫治器，以种植体为支抗压低上切牙，通过X线头影测量评价压低效果。结果16例患者的63颗上切牙均被成功压低，疗程平均10．8个月。前牙覆胎由压低前的（4．04±2．14）mm减小到压低后的（1．30±1．36）mm，减少（2．74±2．00）mm（P〈0．001），露龈笑明显改善。X线头影测量显示，上中切牙切缘到腭平面的垂直距离由压低前的（31．25±3．28）mm，减小至压低后的（26．25±3．25）mm，减少（5．00±2．76）mm，压低前后的差异有统计学意义（P〈0．001）；上中切牙相对于腭平面的唇向倾斜度由压低前的（103．97±8．06）。增加到压低后的（113．30±10．61）。，增加（9．34±10．06）。，压低前后的差异有统计学意义（P〈0．01）。结论应用种植体为支抗压低上切牙改善露龈笑是可行且有效的。Objective To evaluate the effectiveness of mini-screw anchorage on intruding upper incisors in patients with gummy smile. Methods Sixteen female patients （ mean age： 23.8 years） were involved in this study. The mini-screws were placed between upper lateral incisors and canines on both sides for intruding upper incisors together with fixed appliance. The cephalometric analysis was carried out. Results Totally 63 teeth in 16 patients were intruded successfully and the deep overbite was decreased by （2. 74 ± 2.00） mm（ P 〈 0. 001 ）. The distance from the edge of the upper incisor to palatal plane was reduced by （5.00±2.76） mm （ P 〈 0. 001 ）, and the incisors were proclined by （ 9. 34 ± 10. 06 ） ° （ P 〈 0. 01 ）. The duration for intrusion was 10. 8 months on average. Conclusions It was practicable and effective to improve gummy smile by intruding upper incisors with mini-screw implant.
