经椎弓根固定器械治疗不稳定性胸腰椎骨折的临床研究

摘　　要：目的探讨不稳定性胸腰椎骨折的治疗方法。方法采用经椎弓根固定器械（C-D器械）固定,加选择性椎间或椎体内植骨的方法治疗不稳定性胸腰椎骨折。结果术后93例达到解剖复位,3例大部分复位,1例严重压缩性骨折,压缩的椎体虽未复原,但恢复了正常的脊柱力线。不全瘫者20例术后均有不同程度的功能恢复,完全截瘫者12例功能无恢复。40例经1-3年随访,畸形矫正满意,无矫正角度丢失,取钉后腰背僵硬感消失。结论该术式复位、固定可靠,不需另取髂骨,创伤小,可最大限度保留未损伤运动节段功能。Objective To explore the treatment of unstable thoracolumbar fractures. Methods The unstable thoracolumbar fractures were treated with pedicle 93 cases got anatomical reduction, 3 cases almost fixation apparatus as well as lumbar interbody fusion. Results got reduction, 1 case got spinal alignment despite the serious compression fracture,20 cases got some fuctional recovery of some degree despite incomplete paraplegia and 12 cases of complete paraplegia got no recovery. The 1-3 years of follow-up surveys showed that 40 cases got satisfactory correction of deformity without any loss of perspective correction or any sense of back stiffness after taking out nails. ConelusionThis kind of treatment can get reliable reduction and fixation without using ilia to produce wounds and can preserve the the functions of motion segments that are not injured.
