摘 要:目的探讨乌司他丁(Ulinastatin,UTI)对急性有机磷农药中毒(AOPP)的免疫保护作用。方法45例重症AOPP患者随机分为Ⅰ组(UTI治疗组,n=23)和Ⅱ组(非UTI治疗组,n=22)。在急性期治疗前后分别测定肿瘤坏死因子-α(TNF—α)、血清细胞间黏附分子-1(ICAM—1)的水平变化,并以20例健康人为对照。结果Ⅰ组TNF—α和ICAM-1治疗前后比较差异均有统计学意义(P〈0.01),Ⅱ组治疗前后比较差异均无统计学意义(P〉0.05),两组治疗后比较差异均有统计学意义(P〈0.01)。结论UTI对AOPP患者在免疫领域中起治疗作用,其最主要机制可能是通过抑制两种关键性的免疫细胞因子TNF—α、ICAM—1对炎性介质的级联“瀑布”效应,终止它们对重要器官的进一步损害。Objective To investigate the protective effects of ulinastatin on acute organophosphorus pesticides poisoning ( AOPP). Methods 45 patients of severe AOPP were randomly divided into ulinastatin intervention group ( group Ⅰ ) and non - ulinastatin intervention group ( group Ⅱ). The changes of TNF - α and ICAM - 1 in whole blood of the groups before and after the treatment were compared. 20 healthy individuals were selected as control group. Results There were significant differences in TNF - α and ICAM - 1 levels before and after the treatment of group Ⅰ ( P 〈 0.01 ). The results were opposite in group Ⅱ ( P 〉 0.05 ). There were significant differences in TNF - etand ICAM - 1 levels between group Ⅰ and group Ⅱ ( P 〈 0. 01 ). Conclusion Ulinastatin had immune protection on AOPP. It may protect major organs from the further injury by inhibiting the expression of TNF - etand ICAM - 1.
