中医辨证结合西医分期治疗类风湿关节炎临床研究  

Clinical Research of the Syndrome Differentiation Treatment of TCM in Combination with Stage-based Treatment of Western Medicine on Patients with Rheumatoid Arthritis

摘　　要：目的观察风湿康冲剂加甲氨蝶呤(MTX)治疗类风湿关节炎(RA)的临床疗效。方法将102例患者随机分为两组,治疗组予以风湿康冲剂加MTX,对照组给予来氟米特加治疗;疗程均为3个月。比较两组治疗后临床症状指标、实验室检测指标变化及不良反应的发生率。结果治疗4周、12周后,两组患者晨僵时间、关节疼痛、关节肿胀、关节压痛等临床症状均有明显缓解,但治疗组均优于对照组;治疗组实验室指标C反应蛋白(CRP)、血沉(ESR)的改善亦较对照组显著;IgG-RF、CCP-Ab治疗组均有明显降低,但与对照组比较无显著差异。患者恶心、呕吐、腹泻、脱发、肝酶升高、白细胞降低等不良反应发生率,治疗组明显低于对照组。结论风湿康冲剂联合MTX可有效控制RA关节炎症,增强临床疗效,并可减轻MTX的不良反应;风湿康冲剂加MTX治疗RA是具有良好前景的治疗方案。Objective： To observe the therapeutic effect of Fengshikang Medicinal Granules combined with MTX in the treatment of rheumatoid arthritis（RA） . Methods： 102 RA patients were divided into 2 groups randomly. The treatment group was treated with Fengshikang Medicinal Granules plus MTX, and the control group was given leflunomide in combination with MTX. The treatment course lasted for 3 months. The clinical symptoms both at pre - treatment and post - treatment were observed and evaluated. The changes of these laboratory parameters were detected and the adverse reaction rate was monitored. Results： After 4 weeks and 12 weeks treatment, the morning stiffness, joint pain, joint swelling, joint tenderness were significantly improved in both group, and the treatment group was superior to the control group. The laboratory parameters reflecting inflammation including CRP and ESP were also significantly improved in comparing with the control group. However, there was no significant difference of IgG - RF and CCP - Ab in both groups. The adverse reaction rate of treatment group was lower than control group. Conclusion： Fengshikang Medicinal Granules in combination with MTX plays effective role in the treatment of RA. Fengshikang Medicinal Granules can enhance the therapeutic effect of MTX as well as decrease its adverse effect. Fengshikang Medicinal Granules in combination with MTX is an ideal method in the treatment of RA.
