缺血性中风急性期中医证型特点及与血浆HCY的关系

摘　　要：目的探讨缺血性中风急性期中医证型分布特点及与同型半胱氨酸(HCY)的关系。方法将186例缺血性中风急性期患者按中医辨证分为风证、火热证、痰证、血瘀证、气虚证、阴虚阳亢证六型,用荧光偏振免疫法检测血浆HCY水平。结果缺血性中风始发状态组中医证型特点是以风、火热、痰证为主,而风、火热、痰证组血浆HCY水平显著高于健康对照组及血瘀、气虚、阴虚阳亢证组。缺血性中风急性期3～4周组中医证型特点是以风、痰、血瘀证为主,而风、痰、血瘀证组血浆HCY水平显著高于健康对照组及火热、气虚、阴虚阳亢证组。结论缺血性中风急性期中医证型特点与HCY水平一致,为中医辨证施治缺血性中风急性期提供了依据,亦证明高HCY血症是缺血性中风的危险因素之一。Objective： To investigate the relationship between characteristics of TCM syndrome and plasma homocysteine（HCY） at acute cerebral ischemic stroke. Methods： 186 cases of acute cerebral ischemie stroke were divided into Feng syndrome, Heat syndrome, Phlegm syndrome, Blood stasis syndrome, Qi deficiency syndrome, Hyperactivity of Yang due to Yin deficiency. The level of HCY in plasma was detected by fluorescenoe polarization immunoassay. Results： The initial condition of TCM syndrome types in cerebral ischemic stroke was usually expressed by Feng syndrome, Heat syndrome and Phlegm syndrome. The level of HCY in these patients was significantly higher than that of healthy controls group, Blood stasis syndrome group, Qi deficiency syndrome group, Hyperactivity of Yang due to Yin deficiency group. The TCM syndrome types＇ distribution characters were usually expressed by Phlegm syndrome and Blood stasis syndrome types in the 3 - 4 weeks of cerebral ischemic stroke. HCY level in these patients was significantly higher than that of healthy controls and the patients with Heat syndrome, Qi deficiency syndrome, Hyperactivity of Yang due to Yin deficiency. Conclusion： The characters of TCM syndrome types in cerebral ischemic stroke are consistent with the HCY level in plasma. It provides basis for differentiation and treatment the cerebral ischemic stroke. Furthermore, the high level of HCY in plasma is a risk factor of cerebral ischemic stroke.
