中国北方汉族人载脂蛋白AII基因-265T/C多态性与2型糖尿病并发冠心病的相关性研究

摘  要:目的探讨我国北方地区汉族人载脂蛋白AII基因-265T/C(APOAII-265T/C)基因多态性与2型糖尿病并发冠心病的关系。方法采用聚合酶链反应限制性片段长度多态性(PCR—RFLP)技术检测了155例对照组、101例2型糖尿病组和78例糖尿病并发冠心病组APOAII-265T/C多态性基因型和等住基因频率分布。分析基因多态性对糖化血红蛋白(HbAlc)、血糖、血脂水平的影响。结果3组研究对象的APOAII-265T/C多态性基因型和等住基因频率分布差异无统计学意义。不同基因型各组间血脂水平、HbAlc、血糖存在-定差异,不论C住基因携带者还是TT基因型,糖尿病并发冠心病患者TC均低于对照组和糖尿病组,而TG,GLU和HbAlc均高于对照组,同时LDL—C,GLU和HbAlc水平均低于糖尿病组。未发现APOAII-265T/C与性别、高血压家族史以及肥胖相关。Logistic回归分析显示年龄、HbAlc是糖尿病并发冠心病的危险因素,HDL—C则是糖尿病并发冠心病的保护因素[B=-0.649,Exp(B)=0.785,95oACI=0.287~2.148,P=0.02]。结论APOAII-265T/c多态性与糖尿病并发冠心痛无明显关联性,不是我国北方地区汉族人糖尿病并发冠心病发病的独立危险因素。Objective To explore the effect of the interaction between APOAII-265T/C on the presence of polymorphism and type 2 diabetes mellitus(DM) with coronary heart disease in Han nationality from northern China. Methods Geno- types of APOAII-265T/C polymorphisms of APOAII gene were analyzed by using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) in 155 unrelated healthy individuals, 101 patients dignosed of type 2 DM ,and 78 patients of type 2 DM combined with coronary heart disease (CHD). The relationship of gene polymorphisms of APOAII-265T/C and levels of HbAlc,serum lipids,and glucose was also analysed. Results Neither the frequencies of genotypes nor frequencies of alleles of APOAII gene -265T/C polymorphisms were statistically different among DM pa- tients, DM+CHD patients and controls (P〉0.05). Serum lipid levels ,glucose and HbAlc was presented differently in the genotype subgroups. Regardless of C allele gene carriers or TT genotype,in the DM+CHD group,TC levels were lower than that of control group and DM group, and levels of TG,GLU and tlbAlc were higher than the presence in the control group,while levels of LDL-C,GLU and HbAlc were lower than that in DM group. No relationship between gender,family history of HBP,BMI and APOAII gene -265T/C polymorphisms was found. Logistic regression analysis showed that age and HbAlc was the risk factor ,but HDL-C was a protective factor[β= -0. 649,Exp (6)= 0. 785,95 %CI= 0. 287- 2. 148, P =0.02]for type 2 DM with coronary heart disease. Conclusion The -265T/C polymorphisms of APOAII gene may not the major genetic risk factors of type 2 DM with coronary heart disease in Han nationality.
