高容量血液滤过对重症腹腔感染术后患者急性呼吸窘迫综合征的治疗研究

摘　　要：目的探索高容量血液滤过对腹腔感染术后脓毒症患者急性呼吸窘迫综合征的治疗作用。方法选择湖南省人民医院普通外科44例因腹腔重症感染术后ARDS并行呼吸机治疗的患者,其中常规综合治疗组25例,HVHF治疗组19例,观察两组患者氧合指数、动脉血气指标、肺顺应性及APACHE-Ⅱ评分。结果19例患者经HVHF治疗后氧合指数及肺顺应性改善,APACHE-Ⅱ也有明显下降,差异均有显著性(P<0.01)。经多次治疗后,最终患者APACHE-Ⅱ评分明显下降,差异有显著性(P<0.05)。结论HVHF可以辅助改善ARDS患者的呼吸功能,缓解病情。[Objeetive]To evaluate the clinical efficets of high-volume hemofiltration （HVHF） on treating severe abdominal infection patients with postoperative acute respiratory distress syndrome （ARDS）. [Methods] Forty-four patients with ARDS supported by mechanical ventilation were analyzed. First group （Group I） underwent conventional treatment, and second group （Group II） was treated with HVHF. The differences of oxygenation indices, pulmonary dynamic compliance, APACHE- II scores before and after HVHF therapy were analyzed among the two groups. [Results] After the HVHF treatment, the oxygenation indicates and pulmonary dynamic compliance improved, and the APACHE- II scores also decreased significantly （P 〈0.05）. After several HVHF treatments, the APACHE- II scores also showed a significant decreased （P 〈0.05）. [Conclusion] HVHF can improve both the respiratory function and vital signs for patients with ARDS caused by severe abdominal sepsis, and had a better outcome.
