乳腺癌超声造影特点、中医证型与Ki-67表达的相关性研究

摘　　要：目的探讨乳腺癌患者超声造影肿瘤微血管灌注特点与中医辨证分型、分子生物学指标Ki-67蛋白之间的相关性。 方法回顾性观察分析175例确诊为乳腺癌的术前超声造影检查、中医证型分类,术后标本应用免疫组织化学染色测定乳腺癌原发灶癌细胞Ki-67蛋白的表达情况,并分析与超声造影检查、中医证型之间的关系。 结果175例中肝郁痰凝型者98例（56.0%）,冲任失调型64例（36.6%）,正虚毒炽型13例（7.4%）。超声造影灌注特点、肿瘤临床分期与Ki-67阳性表达呈正相关。正虚毒炽组出现Ki-67蛋白阳性的几率最高,冲任失调组次之,肝郁痰凝组最低。 结论乳腺癌超声造影肿瘤微血管灌注特点与中医证型、分子生物学指标Ki-67蛋白之间存在一定的相关性。乳腺癌的肿瘤微血管灌注特点从一定程度上反映了癌细胞的生物学行为和预后,为临床治疗方案的选择提供客观依据。Objective To investigate the correlation among contrast-enhanced ultrasonography （CEUS） blood perfusion features,syndrome differentiation of traditional Chinese medicine （TCM） and Ki-67 antigen of the breast cancer.Methods The perfusion features and syndrome differentiation of 175 cases of breast cancer confirmed by histopathology were analyzed retrospectively.The expression of Ki-67 in the mastectomy specimens of the breast cancer was determined by immunohistochemistry.The relationship among these CEUS characteristics,syndrome differentiation and Ki-67 expression were analysed.Results 175 patients who suffered from breast cancer were grouped into three syndromes.The proportion of stagnation of liver-qi and phlegm type,Chong and Ren meridians disordered type,and zhengqi-weak-evil-strong type was 56%,36.6%,and 7.4% respectively.The expression level of Ki-67 antigen was closely correlated with the CEUS perfusion features and TCM stage.Ki-67 expression was the highest in zhengqi-weak-evil-strong type.Chong and Ren meridians took the second place.It was the lowest in stagnation of liver-qi and phlegm type.Conclusions There are relative correlations among CEUS features,syndrome differentiation of TCM and Ki-67 expression in breast cancer.The biologic behavior and prognosis can be assessed according to CEUS features,which provides the basic of facts for clinical cure.
