体外受精-胚胎移植前超声介入硬化治疗输卵管积液的价值

摘　　要：目的探讨超声硬化治疗输卵管积液在体外受精-胚胎移植（IVF-ET）中的价值。 方法IVF-ET前应用经阴道彩色多普勒超声（TVCD）对33例输卵管积液不育患者进行硬化治疗,并与19例未治疗输卵管积液患者及47例单纯性输卵管梗阻患者的IVF-ET结果进行比较。 结果超声硬化治疗后,27例患者积液完全消失,6例患者最大无回声区面积较治疗前减少90%以上,有效率达100%。99例患者共实施IVF-ET107个周期,治疗组（35个周期）促性腺激素（Gn）的使用时间、用量、获卵数、成熟卵子数、受精率、卵裂率、优质胚胎率、胚胎种植率、临床妊娠率与未治疗组（22个周期）和对照组（50个周期）间无显著性差异;而胚胎种植率和临床妊娠率较未治疗组（22个周期）显著增高（P〈0.05）。 结论TVCD硬化治疗输卵管积液操作简便、安全、有效;IVF-ET前实施硬化治疗可显著提高临床妊娠率。Objective To evaluate the effect of ultrasound sclerotherapy for women with hydrosalpinx fluid before in vitro fertilization and embryo transfer（IVF-ET）.Methods Ultrasound sclerotherapy by transvaginal color Doppler sonography（TVCD） was applied to 33 infertile women with hydrosalpinx fluid before IVF,and the outcomes of IVF-ET （treated group） were compared with 19 untreated hydrosalpinx fluid women（untreated group） and 47 women with simple tubal obstruction （control group）.Results In the treated group hydrosalpinx fluid disappeared in 27 cases and the amount of fluid decreased above 90% in 6 cases.100% effective rate was reached after ultrasound sclerotherapy.The patients in all groups were performed IVF-ET.It was found that there was no significant difference in gonadotrophin（Gn）time and dosage,rates of fertilization,cleavage,high-quailty embryo,embryo implantation and clinical pregnancy among the treated group （35 cycles）,the untreated group（22 cycles）and the control group （50 cycles）,but the rates of embryo implantation and clinical pregnancy in the treated treatment group were significantly higher than those in the untreated group.Conclusions The sclerotherapy of hydrosalpinx fluid with TVCD is convenient,safe and effective in practice and could significantly increase the pregnancy rate of IVF-ET.
