基质金属蛋白酶-9对急性冠状动脉综合征患者近期预后的判断价值

摘　　要：目的观察基质金属蛋白酶-9(MMP-9)、超敏C-反应蛋白(hs-CRP)、心肌肌钙蛋白I(cTnI)在急性冠状动脉综合征(ACS)的变化,探讨MMP-9对ACS近期预后的判断价值。方法分别测定60例ACS、30例稳定型心绞痛及30例健康成人MMP-9、hs-CRP、cTnI水平,随访3个月观察ACS心脏事件发生率。结果MMP-9浓度,ACS组(30.33±14.00)ng/ml高于稳定型心绞痛组(2.72±0.98)ng/ml及正常对照组(2.84±0.82)ng/ml(P<0.05);hs-CRP浓度,ACS组(8.05±2.48)mg/L高于稳定型心绞痛组(2.27±0.84)mg/L及正常对照组(2.17±0.91)mg/L(P<0.05);cTnI浓度,ACS组(0.78±0.21)ng/ml高于稳定型心绞痛组(0.29±0.09)ng/ml及正常对照组(0.29±0.08)ng/ml(P<0.05)。60例ACS随诊期间,37例MMP-9浓度高于30 ng/ml,其中18例发生心脏事件,阳性率48.6%,23例MMP-9浓度低于30 ng/ml,其中3例发生心脏事件,阳性率13.0%,2组差异有显著性(P<0.05)。结论MMP-9、hs-CRP、cTnI水平在ACS组中明显升高,近期随访结果提示MMP-9与ACS的近期预后有关,可作为预后的判断指标。Objective To study the serum levels of matrix metalloproteinase-9 （ MMP-9 ）, high senstivity C-reactive protein （hs-CRP） and cardiac troponin I（cTnI）in patients with acute coronary syndrome （ACS）and its short-term prognostic value. Methods The study population included 60 patients with ACS, 30 patients with stable angina pectoris （SAP） and 30 healthy adults who attended a routine health check （control group）. With the pinpoint of the MMP-9 〉 30ng/ml,the two groups were observed of the rate of cardiac events. Results Serum levels of MMP-9 were significiently higher in the ACS group （ 30. 33 ± 14.00） ng/ml than those in the SAP group（ 2.72 ±0.98 ） ng/ml and the control group（2.84 ± 0.82 ） ng/ml（ P 〈 0.05）. The SAP group had no difference with the control group （ P 〉 0.05 ）. Serum levels of hs-CRP were significently higher in the ACS group （ 8.05 ± 2.48 } mg/L than those in the SAP group （ 2.27 ± 0.84 ） mg/L and the control group （ 2.17 ± 0.91 ） mg/L （ P 〈 0.05 ）. The SAP group had no difference with the control group （ P 〉 0.05 ）. Serum levels of cTnI were significiently higher in the ACS group （ 0.78 ± 0.21 ） ng/ml than those in the SAP group （ 0.29 -± 0.09 ） ng/ml and the control group （ 0.29 ± 0. 08） ng/ml（P 〈 0.05 ）. The SAP group had no difference with the control group（ P 〉 0.05 }. In the three-month follow-up,37 patients with higher MMP-9 in the 60 ACS had 18 cardiac events. The ratio of cardiac events were higher than that in the control group （ P 〈 0.05 ）. Conclusion The MMP-9 plays important roles in the prognosis in patients with ACS. MMP-9 could be used as an indicator of risk stratification. The united detection of MMP-9, hs-CRP and cTnI is more meaningful in ACS patients.
