摘 要:目的观察并探讨异丙酚对家兔全脑缺血再灌流(I/R)损伤的保护作用及其机制。方法24只家兔随机分为假手术组、I/R组和异丙酚组,每组8只。分别在缺血前15min(I0)及再灌流30min(R1)、2h(R2)和4h(R3)取颈内静脉血,测定血浆内皮素-1(ET-1)、丙二醛(MDA)和过氧化物歧化酶(SOD)的含量,实验结束取皮层HE染色,光镜观察,并测量神经元核截面积。结果I/R组及异丙酚组ET-1均升高,以I/R组更甚,R3时两组值与假手术组比较,差异均有统计学意义(t分别=3.52、2.81,P均<0.05),异丙酚组与I/R组比较,差异也有统计学意义(t=3.15,P<0.05);I/R组各点MDA较假手术组相应值明显升高,差异均有统计学意义(t分别=3.35、3.27、3.36,P均<0.05);异丙酚组MDA无明显变化,但与I/R组相应时点比较,差异均有统计学意义(t分别=2.16、2.51、2.55,P均<0.05);异丙酚组SOD比假手术组降低,但与I/R组比较同期明显增加,差异有统计学意义(t分别=2.75、2.53、2.21,P均<0.05)。异丙酚组核截面积比I/R组明显增加[(189.7±19.06)μm2vs(105.63±16.21)μm2];光镜下异丙酚组皮层轻度水肿、部分尼氏体溶解消失;而I/R组皮层水肿明显,大量神经元坏死。结论异丙酚不仅增强机体抗氧化能力,而且还能抑制ET-1的合成和释放,阻断氧自由基和ET-1之间的恶性循环,从而保护脑I/R损伤。Objective To investigate the protective effects of propofol on rabbit cerebral ischemia-reperfusion (I/R) injury and its mechanism. Method Twenty-four rabbits were divided randomly into sham-operation group, I/R group and propofol group. Blood samples of each rabbit were obtained respectively, through jugular vein access, 15 min before cerebral ischemia model was established (10); and 30 min (R1), 2h (R2) and 4h (R3) after reperfusion. The levels of plasma endothelin-1(ET-1), malondialdehyde (MDA) and superoxide dismutase (SOD) were measured. At the end of the experiment, hematoxylin and eosin (HE) staining for cortex was processed and observed under microscope, and the cross-section of the neuron nucleus was measured. Results The level of ET-1 was higher in the propofol group and the I/R group than the sham-operation group, and the difference was statistically significant at R3 point (t=3.52,2.81 ,P〈0.05); and between the propofol group and I/R group, the difference also was statistically significant (t=-3.15, P〈0.05). The level of MDA in I/R group was higher than sham-operation group at every point and there were significant differences (t=3.35, 3.27, 3.36, P〈0.05). There was no obvious change in the level of MDA in the propofol group, however, the difference between propofol group and I/R group was also significant (t = 2.16,2.51,2.55, P〈0.05) at all points (R1, R2, R3). The level of SOD in propofol group was reduced compared with sham-operation group, but it was remarkably increased compared with I/R group and there were statistical differences at all points (t=2.75, 2.53, 2.21, P〈0.05). The cross-section of the neuron nucleus in the propofol group increased obviously, compared with that in the I/R group (189.7±19.06um^2 vs. 105.63±16.21um^2).
