全身麻醉下双气囊电子小肠镜检查的护理配合

摘　　要：目的:通过静脉全身麻醉下对不明原因小肠疾病患者行双气囊推进式电子小肠镜检查,评价其安全性,总结护理要点。方法:对103例有消化道症状,经胃镜、结肠镜、全消化道钡餐、腹部血管造影等检查无阳性发现且无肠粘连及严重脏器功能不全的患者,在静脉麻醉状态下,应用双气囊推进式电子小肠镜检查,视患者病情决定经口侧或经肛侧检查。结果:103例患者插镜成功率100%,其中49例行口侧检查,31例行肛侧检查,另有23例行双侧检查。检查过程中均未出现明显并发症。结论:双气囊推进式小肠镜可直视检查全小肠腔病变,在麻醉状态下进行检查,并辅以适当的护理配合,能够成功完成检查,是一种安全、可靠的诊断和治疗小肠疾病的方法。Objective To perform the double - balloon smaU intestinal endoscopy for the cause - unclear small intestinal diseases under intravenous anesthesia, to evaluate the safety and summarize the nursing points. Method Total 103 patients with the gastrointestinal symptoms from October 2007 to March 2009 received the routine examinations such as gastroscopy, colonoscopy,barium meal examination of digestive tract, and abdominal digital subtraction angiography （DSA）. There was no positive result, and no cases with intestinal conglutination or severe organ dysfunction. These patients were performed double - balloon small intestinal endoscopy under intravenous anesthesia. Whether the oral route or the anal route was used depended on the clinical conditions of patients. Resillts The successful rate of inserting the small intestinal endoscopy was 100%, in 49 cases were examined from the oral route only,31 cases from the anal route only,and the other 23 cases were examined from both routes. There was no obvious complication during the examination. Conclusion The double - balloon small intestinal endoscopy can directly visualize the lesions of the small intestinal cavity. The examination under the intravenous anesthesia, accompanies with proper nursing cooperation, can be accomplished successfully. It is a safe, reliable method in diagnosing and treating the small intestinal diseases.
