白癜风自体表皮移植术后移植区遗留白斑复色规律的研究

摘　　要：目的:观察白癜风自体表皮移植术后移植区遗留白斑的复色规律。方法:利用负压吸疱自体表皮移植术治疗白癜风,并在术后痂皮完全脱落后开始行窄谱UVB照射,在临床复诊时摄像并记录术后移植区遗留白斑大小的变化。结果:接受自体表皮移植术资料完整的白癜风患者33例,共接受手术移植的皮损为141片,移植后受皮区炎症消退后共出现内部遗留白斑1 179片,其中面部411片,躯干768片;外部遗留白斑1 836片,其中面部768片,躯干1 068片。在内部、外部遗留白斑中,面部皮损的总有效率(97.57%,94.92%)均明显高于躯干部位的皮损(83.33%,79.21%)(P均<0.05)。白斑宽度在相同范围时,面部内部和外部白斑的有效率分别大于躯干部(P均<0.05);在面部和躯干部白斑中,宽度≤1mm的内、外部白斑的有效率均分别大于该部位宽度>1mm的内、外部白斑(P均<0.05);当白斑宽度在相同范围时,面部内部遗留白斑的有效率与该部位外部白斑没有明显的差异(P>0.05);当白斑宽度≤1mm时,躯干部内部遗留白斑的有效率与该部位外部白斑没有明显的差异(P>0.05),当白斑宽度>1mm时,躯干内部白斑的有效率大于该部位外部白斑(P<0.05)。结论:白癜风自体表皮移植术术后移植区遗留白斑复色与遗留白斑所在部位、遗留白斑的位置和遗留白斑面积密切相关。Objective To study recovery rules of white spots left on the transplantation area after autologous epidermal grafting in vitiligo. Methods 33 patients with vitiligo were treated with autologous epidermal grafting, and then coped with narrow UVB after the scars peeled off naturally. The patients were followed up by photograph and recording the size of white spot left on the transplanted area. Results Of the 33 patients, 141 sheets of vitiligo lesions were applied the operation and there were 1179 sheets of inner white spots （face ： 411 sheets,trunk ：768 sheets） and 1836 sheets of outer white spots （face ： 768 sheets, trunk：1068 sheets） left on the transplanted area. Among the inner and outer white spots, the total effective rate of facial lesions were higher than that of trunk （P〈0.05）, respectively. When the size of white spots was in the same range, the effective rate of inner and outer facial were higher than that of trunk, respectively （P〈 0.05）. The effective rate of inner and outer white spots （≤1mm） is higher than the lesions （〉1mm） at the same lacation, respectively （P〈0.05）. When the width was in the same range, there were no significant differences of effective rates between the inner and outer facial white spot （P〉0.05）. When the width was less than lmm, there were no significant differences of effective rates between the inner and outer white spot on the trunk （P〉0.05）. When the width was larger than lmm, the effective rate of inner white spots of the trunk was higher than that of the outer parts （P〈0.05）. Conclusion The recovery of color after autologous epidermal grafting in vitiligo is related to the position, outer and inner location and the size of white spots left.
