我院《抗菌药物临床规范使用实施细则》试行前后普外科应用抗菌药物的调查分析

摘　　要：目的:分析我院制定《抗菌药物临床规范使用实施细则》(简称《细则》)前后,普通外科治疗结节性甲状腺肿中抗菌药物的应用和《细则》执行的情况。方法:回顾性调查2005年11月-2007年10月间结节性甲状腺肿患者抗菌药物的应用情况,对规范前组(2005年11月-2006年10月)和规范后组(2006年11月-2007年10月)数据进行统计分析。结果:《细则》实施前后,患者的住院时间由8.47d缩短为8.31d,抗菌药物使用频率较高的为头孢呋辛、依替米星和舒巴坦钠等,平均使用天数由4.77d缩短为3.78d;术前用药率由76.26%增加到83.42%,用药时机恰当;抗菌药物的联用较少。结论:实施《细则》后,我院普外科单病种抗菌药物的应用基本符合规范,但规范前后改进的效果不太明显,应在全院范围内推广此《细则》,以进一步加强对抗菌药物的规范使用。Objective： To analyze the application of antibacterial drugs in curing nodular goiter in surgery department before and after the implementation of 〈 Detailed Rules of Standard Application of Antibacterial Drugs in Clinic 〉. Methods： The antibacterial drugs used for the thyroidectomy patients were reviewed from November, 2005 to October, 2007. And the data of the pre-standardized group and the standardized group were statistically analyzed. Results： Before and after the implementation of the detailed rules, the average hospital stay of the patients was shortened from 8.47 days to 8.31 days. The antibacterial drugs used high frequenoily were cefuroxime, etimicin, sulbactam sodium, which were almost not in accordance with the detailed rules, and the average time of the antibacterial drugs used was shortened from 4.77 days to 3.78 days. The rate of the antibacterial drugs used before operation raised from 76.26% to 83.42% and the occasion of the drug use was appropriate. The drug combination of antibacterials was not common. Conclusion： The utilization of antibacterial drugs was in accordance with the detailed rules after the implementation of the detailed rules, but the effect was not obvious. The detailed rules should be popularized in our hospital to strengthen the normative utilization of antibacterials.
