两种不同材料固定Mayo Ⅲ型尺骨鹰嘴骨折效果比较

摘　　要：目的:比较可吸收材料张力带和克氏针钢丝张力带固定MayoⅢ型尺骨鹰嘴骨折的疗效。方法:采用两种材料相同方法治疗MayoⅢ型尺骨鹰嘴骨折45例,可吸收材料张力带21例,克氏针钢丝张力带24例,比较其疗效。结果:按Morrey和Broberg评分标准,可吸收材料张力带组优13例,良4例,可4例,差0例,优良率为81.0%;克氏针钢丝张力带组优15例,良5例,可4例,差0例,优良率为83.3%。可吸收材料张力带组石膏固定时间明显长于克氏针钢丝张力带组,在手术时间、出血量、功能恢复及优良率方面差异无显著性(P>0.05)。结论:可吸收材料和克氏针钢丝张力带都是固定MayoⅢ型尺骨鹰嘴骨折的可效方法。可吸收材料固定免去第二次手术,减少患者痛苦,对于非过敏体质患者,可以推广使用。Objective To compare the effects of absorbable material tension band with Kirschner wire tension band in treating MayoⅢ type olecranon fractures. Methods 45 cases with olecranon fractures were treated with absorbable material tension band （21 cases, absorbable material group） or Kirschner wire tension band （24 cases, Kirschner wire group）, and the effects were compared. Results According to Broberg and Morrey elbow rating system, 13 cases in absorbable material group were excellent, 4 cases were good, 4 case were fair, and no case was poor （the excellent and good rate was 80.9%）; 15 cases in Kirschner wire group were excellent, 5 cases were good, 4 cases were fair, and also no case was poor （the excellent and good rate was 83.3%）. The duration of plaster immobilization in absorbable material group was longer than that in Kirschner wire group. There were no significant differences of operating duration, blood losing, function and Mayo elbow performance index between the two groups. Conclusions Both absorbable material tension band and Kirschner wire tension band are effective in treating MayoⅢ olecranon fracture. But absorbable materials can avoid the second time of operation to remove internal materials, so to release the suffering of patients. It is advisable to use absorbable material tension band in non-hypersusceptibility patients with MayoⅢ type olecranon fractures.
