超声检查在妇产科急腹症中的诊断价值

摘　　要：目的探讨彩色多普勒超声检查对妇产科急诊的诊断与鉴别诊断价值及对临床治疗的指导意见。方法回顾性总结我院2年中225例妇产科急腹症患者的超声检查资料,与手术及病理诊断相比较分析,并对非手术治疗的病例进行随访观察,了解其治疗效果。结果225例中超声诊断符合210例,总符合率93.3%。其中异位妊娠112例,黄体破裂16例,卵巢囊肿蒂扭转8例,急性盆腔炎25例,不全流产29例,子宫穿孔1例,符合率分别为91.1%,87.5%,87.5%,96.0%,96.6%;前置胎盘25例,胎盘早剥1例,妊娠合并输尿管结石2例,妊娠合并子宫浆膜下肌瘤蒂扭转1例,经血潴留5例,符合率均为100%。结论超声对妇产科急诊能够提供诊断依据,对部分保守治疗患者的随访观察也具有一定临床价值。Objective To discuss the diagnosis value of Colored Doppler for emergent cases in department of gynecology and obstetrics. Methods The ultrasonic diagnostic data of 225 patients with gynecological acute abdomen for 2 years in our hospital were retrospectively analyzed and compared with those of operation and pathological diag- nosis ;follow- up was made to receive no surgical treatment so as to study the curative effect. Results Of 225 cases, 210 cases diagnosed ultrasonically as acute abdomen, coincidence rate was 93.3 % , including 112 cases of ectopic pregnancy, 16 cases of corpus luteum breaking, 8 cases of pediculotors of ovarian cyst,25 cases of acute pelvic in- flammation ,29 cases of incomplete abortion, 1 case of perforation of uterus. The diagnostic rates were 91.1% ,87. 5% ,87.5% ,96.0% ,96.6% ;25 cases of placenta praevia, 1 case of placental abruption,2 cases of pregnancy combined with calculus of ureter, I case of pediculotorsion of uterus subserous myoma,5 cases of retention of men- ses. The diagnostic rates were all 100%. Conclusion Uhrasonography plays an important role in diagnosis of gyne- cologic emergency ; It is of certain clinical value for those cases receiving expectant treatment and in follow - up.
