摘  要:目的探讨逆行胫后动脉穿支皮瓣修复足跟皮肤软组织缺损的可行性。方法术前以彩色超声多普勒探测标记胫后动脉穿支穿出部位,以该点为旋转点设计逆行胫后动脉穿支皮瓣修复足跟皮肤软组织缺损7例。结果7例皮瓣全部成活,皮瓣受区和供区创口一期愈合,术后随访3-6个月.皮瓣及其蒂部外形不臃肿.色泽、质地正常相近。结论逆行胫后动脉穿支皮瓣不携带深筋膜和胫后动脉,对皮瓣供区影响小,蒂部只保留穿支血管,便于旋转,皮瓣薄,修复足跟外形恢复好,是修复足跟皮肤软组织缺损较为理想的方法之一。Objective To introduce a new converse flap based perforator of posterior tibial artery to repair skin and soft tissue defects in heel. Methods Seven cases with skin and soft tissue defects in heel had been repaired by the flap based perforator of posterior tibial artery, located and signed it by color doppler flow imaging(CDFI) before operation, then designed the converse flap based above perforator sign from January 2007 to March 2008. Results All flaps were survived completely and the wounds got primary closure in 7 cases, the flaps were not overstaffed, their shape, texture and color were similar to normal, all of the 7 cases were evaluated as satisfactory after 3 months to 6 months follow-up. Conclusion The converse flap based perforator of posterior tibial artery is one of ideal flaps for repairing skin and soft tissue defects in heel because it doesn't sacrifice deep fascia and posterior tibial artery, doesn't need to make thin by the second operation and can prevent press and overstaff of the flap's pedicel.
