肾衰养真颗粒治疗慢性肾脏病4、5期并发营养不良的临床观察

摘　　要：目的:初步评价肾衰养真颗粒改善慢性肾脏病(CKD)4、5期并发营养不良的有效性和安全性。方法:将130例CKD4、5期并发营养不良患者随机分为3组:颗粒治疗组50例,开同对照组42例和汤剂对照组38例。各组采用基础治疗基础上分别加肾衰养真颗粒、开同、肾衰养真配方汤剂。观察治疗前后的主症评分及SGA评分,血清白蛋白(Alb)、血红蛋白(Hb)、转铁蛋白(Tf)、前白蛋白(PA)、胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、红细胞(RBC)计数、网织红细胞绝对值(RET绝对值#)以及24h尿蛋白定量(24hUpr)、血肌酐(Scr)、尿素氮(BUN),不良反应率等指标。结果:肾衰养真颗粒可改善CKD4、5期并发营养不良患者的营养状况,能升高Alb、Hb、Tf、PA、IGF-Ⅰ水平(P<0.01),增加RBC计数以及RET绝对值#(P<0.01),降低主症评分及SGA评分(P<0.01);并可以减少24hUpr,降低Scr、BUN水平(P<0.01)。结论:肾衰养真颗粒能有效地改善CKD4、5期并发营养不良患者营养状况。Objective：To preliminary evaluate the clinical efficacy and safety of Shenshuai Yangzhen Granule in improving the Chronic Kidney Disease complicated by malnutrition period of four or five. Methods： 130 cases of the Chronic Kidney Disease complicated by malnutrition period of four or five were divided into three groups randomly：50 cases in Granule treatment group, 42 cases in Ketosteril control group and 38 cases in decoction control group. The groups were treated with the basal therapy and Shen- shuai Yangzhen Granule, Ketosteril, and Shenshuai Yangzhen Decoction respectively. Before and after the treatment, the scores of main symptom and SGA were measured. The serum albumin, pre - albumin, transferrin, hemoglobin, insulin - like growth factor - Ⅰ , red blood cells and absolute reticulocyte were tested. The 24 - hour urine protein, serum creatinine, urea nitrogen and the incidence rate of adverse effects were also studied. Results： Shenshuai Yangzhen Granule could improve the malnutrition status of CKD 4 or 5 period patients, improve the level of serum albumin, pre- albumin, transferri, hemoglobin, insulin - like growth factor- [ （P 〈 0.01 ）, increase the red blood cell count, absolute reticulocyte, decrease the scores of main symptom and SGA（P 〈 0.01 ） and reduce the level of 24 - hour urine protein, serum creatinine and urea nitrogen（P 〈 0.01 ）. Conclusion： Shenshuai Yangzhen Granule could safely and effectively improve the malnutrition status of CKD 4 or 5 period patients.
