摘 要:目的探讨慢性乙型病毒性肝炎(CHB)患者内毒素lipo-polysaccharide,LPS)对血清可溶性Fas(souluble Fas,sFas)表达的影响。方法采用随机对照、单盲方法进行前瞻性试验,微生态制剂干预试验。120例患者在常规护肝治疗的基础上,随机分成口服思连康(治疗组)60例,不服思连康(对照组)60例,观察治疗前后血清LPS、sFas、TNF-α和肝功能指标的变化以及它们之间的关系。结果CHB组血清LPS、sFas、TNF-α水平均明显高于正常对照组(t=2.789,t=2.845,t=2.911,P均<0.05);CHB轻度组血清LPS、sFas、TNF-α水平明显低于中度、重度组(t=2.011,t=2.145,t=2.278,t=2.298,t=2.310,t=2.245,P均<0.05);治疗后治疗组LPS、sFas、TNF-α、ALT、TBi水平明显低于对照组(t=2.031,t=2.215,t=2.178,t=2.198,t=2.410,P均<0.05),治疗组治疗前后LPS、sFas、TNF-α、ALT、TBi水平比较有显著差异(t=2.653,t=2.536,t=2.901,t=2.716,t=2.683,P均<0.01);治疗组治疗前后血清LPS、sFas、TNF-α水平与ALT、TBi水平均呈正相关(r=0.518,r=0.535,r=0.502,t=2.111,t=2.045,t=2.168;r=0.542,r=0.581,r=0.616,t=2.211,t=2.275,t=2.178;P均<0.05)。结论LPS、sFas、TNF-α参与调节CHB肝细胞凋亡,抗内毒素治疗可降低血清LPS、TNF-α水平,抑制血清sFas表达,改善肝脏功能。Objective To discuss effects of lipo-polysaccharide(LPS) and serum soluble Fas(sFas) in patents with chronic hepatitis B(CHB).Methods A prospective,randomized,single blind and parallel control clinical study enrolled 120 cases with conventional basis,randomly divided into oral Bifidobacterium tetravaccine Tablets(Live)(treatment group) 60 cases,non-oral Bifidobacterium tetravaccine Tablets(Live)(control group) 60 cases were observed before and after treatment serum LPS,sFas,TNF -α and liver function changes as well as the relationships between them. Results The contents of LPS, sFas,TNF - α were significantly higher than that of the normal control group ( t = 2. 789, t = 2. 845, t = 2. 911, P both 〈 0. 05 ), The contents of LPS, sFas, TNF - α in CHB mild were significantly lower than that of the moderate and severe group ( t = 2. 011, t = 2. 145, t = 2. 278, t = 2. 298, t =2.310, t =2.245, Pboth 〈0.05) ; The contents of LPS,sFas,TNF - α,ALT,TBi CHB after treatment were significantly lower than that of the control group ( t = 2. 031, t = 2. 215, t = 2. 178, t = 2. 198, t =2.410, Pboth 〈0.05);The contents of LPS,sFas,TNF-α and ALT,Tbi in pre -treat- ment were positively correlated ( r =0.518, r =0.535, r =0.502, t =2. 111, t =2.045, t =2. 168, P both 〈 0. 05);The contents of LPS, sFas ,TNF-α and ALT,TBi in post- treatment were positively correlated (r =0. 542, r =0. 581, r =0. 616, t =2. 211, t =2. 275, t =2. 178, P both 〈0. 05). Conclusion The contents of LPS, sFas,TNF - α involved in the regulation of hepatocyte apoptosis, The therapy of anti - endotoxin can inhibit sFas expression and improve liver function.
