嵌入肝内型胆囊切除术治疗体会

摘　　要：报告胆囊床嵌入肝内型胆囊炎、胆囊结石２６例，男１１例，女１５例，年龄４８～８２岁。胆囊床嵌入１～３ｃｍ者１８例，嵌入３～５ｃｍ者７例，嵌入５ｃｍ以上者１例。２６例均依胆囊大部分切除，残余胆囊壁粘膜刮除，碘酒、酒精烧灼。残腔大者，带蒂大网膜填塞、缝合固定治愈。强调此术式创伤小、手术时间短、大大降低医源性损伤。26 cases of cholecystitis cholelithiasis with cholecystic bed embedded into liver were reported,11 males and 15 females,aged from 48 to 82 years old.As to the depth of the embedded parts,18 cases were of 1 to 3 cm,7 cases of 3 to 5 cm,and 1 case of more than 5 cm.ALL of the 26 cases were the type of cholecystis mainly excised,with the residual cholecystic wall mucosa smoothed by curettage and burnt with alcohol ,iodize alcohol.For big residual cavity,fill it with pedicled omentum,healing by suture.Featuring in trauma small,operation period thort,this operation method will largely reduce the medicalgenic damage and benefit the patients to get fine after the operation.
