多囊卵巢综合征患者阴道超声下卵巢的变化与临床表现及内分泌变化的关系

摘　　要：目的：明确是否卵巢呈多囊性改变患者阴道超声检查卵巢的变化与临床、内分泌之间具有相关性。方法：对８６例月经稀发和闭经、阴道超声下卵巢呈多囊改变患者进行了观察，放免法测定了血清中黄体生成素、卵泡刺激素、泌乳素、睾酮，估计了体重指数、多毛情况。部分患者做了糖耐量及胰岛素释放试验。对卵巢的体积、卵泡数与黄体生成素、睾酮、体重指数、多毛程度及胰岛素拮抗指数的相关性做了分析。结果：卵泡数和卵巢体积与睾酮和黄体生成素的水平均有明显的正相关性，Ｔ比ＬＨ相关性更明显，与多毛程度也具有正相关性。胰岛素拮抗程度除与卵巢体积的大小和卵泡数有明显的相关性外，与Ｔ的水平也具有正相关性。结论：阴道超声观察卵巢体积和卵泡数有助于对多囊卵巢综合征的诊断。ABSTRACT Objective: To determine whether ovarian polycystic changes estimated by transvaginal ultrasonography correlate with clinical and endocrinal findings associated with the polycystic ovary syndrome(PCOS). Methods: Eightysix patients suffering from oligoamenorrhea and polycystic ovaries by transvaginal ultrasonography were examined. They were assessed for body mass index (BMI)and hirsutism. Sera were assayed for prolactin (PRL), luteinizing hormone (LH), folliclestimulating hormone(FSH) and testosterone (T) levels. Insulin levels and insulin resistance of 55 from the 86 patients were estimated. Results: Both T and LH levels were correlated significantly with the number of follicles and ovarian volumes, Insulin resistance correlated significantly with ovarian volumes and follicle numbers, and also with T values. Hirsutism was correlated with ovarian volume. Conclusion: Ovarian transvaginal ultrasonography parameters, ovarian volume and follicle numbers, were a valuable additional tool for the diagnosis of PCOS.
