摘 要:目的探讨无脊髓损伤颈椎骨折脱位的牵引加手术治疗方式的选择指征。方法对14例无脊髓损伤颈椎骨折脱位患者,全部采用颅骨牵引加手术治疗。其中前路手术10例,后路3例,前后联合手术1例。结果术后11例获解剖复位,3例稍差。所有患者手术后均达到生活自理并参加一般性工作学习。植骨均获融合。未发生钢板断裂及螺钉松动、断裂等并发症。结论对无脊髓损伤颈椎骨折脱位患者宜行牵引加手术植骨融合内固定治疗。术前经牵引复位者,前路手术为首选;不能经牵引复位者,可据有无椎间盘的损伤而考虑行单纯的后路手术还是行前后联合手术。Objective To study the traction and surgical treatment for cervical spinal fractures and dislocations without spinal cord injury. Methods Fourteen cases of cervical fractures and dislocations without spinal cord injury, were treated with anterior decompression, reduction, autograft ,3 cases with posterior surgery, 1 case with anterior and posterior surgery. Results Clinical manifestations were improved in all cases ,11 cases got anatomical reduction .3 cases was reduces partially.all cases went back to work and 14 cases Intervertebral heights and physiologic curves were kept well ,autograft bone got fusion and there were no plate/screw break or loose. Conclusion Traction and surgery treatment surgery is preferred to cervical spinal fractures and dislocation without spinal cord injury. Through anterior approach, it is feasible to get complete decompression ,good reduction .The cases who could not were reduced through traction,were treated with posterior surgery or anterior-posterior surgery.
