摘  要:目的:探讨鳖甲水煎液对两种不同原因引起的大鼠肝纤维化的预防治疗作用和量效关系,为鳖甲抗肝纤维化的药效物质基础研究及其活性组分抗纤维化新药研发和临床医疗提供科学依据和研究思路。方法:采用CCl4、DMN两种肝纤维化大鼠模型,对鳖甲水煎液抗肝纤维化的药理作用进行较系统研究,考察各组大鼠病理组织学、肝组织胶原含量、羟脯氨酸、免疫组化细胞因子以及TGF-β1信号通道蛋白Smads的表达、血清透明质酸、肝功能、肝组织氧化指标等。结果:鳖甲水煎液组SSS计分、肝组织胶原含量、纤维化指标、促肝纤维化因子、血清酶学指标显著低于模型组,而抗氧化指标显著上升(P〈0.05-0.01),作用相当或优于公认有效的抗肝纤维化药物γ-干扰素、扶正化瘀胶囊,预防组及高剂量组效果尤佳。结论:鳖甲水煎液具有显著的抗肝纤维化预防和治疗综合效应,其作用机制可能是通过抗脂质过氧化、改善肝组织病理、改善肝功能、调控细胞因子水平及信号传导通路等而发挥抑制肝星状细胞(HSC)活化增殖及细胞外基质(ECM)合成分泌、促进ECM降解吸收等综合作用,阻断和治疗肝纤维化。Objective:To investigate the preventive and therapeutic effect of Amydae Carapax decoction on liver fibrosis in rats induced by two different causes and dose-effect relationship, providing scientific evidences and ideas for basic investigation on material basis for its anti-liver fibrosis effect as well as development and clinical application of its active components. Methods:The pharmacological actions of Amydae Carapax decoction on liver fibrosis were observed in CCl4 and DMN induced liver fibrosis in rats. At same time, positive had normal control were used. At the end of animal experiment, semi-quntitative histopathological scores of inflammation and fibrosis, content of collagen of the liver, HYP,levels of liver some eytokines, expression of Smads in TGF - β1 sigualing pathway, serum hyaluronic acid,liver function and liver tissue oxidation index were compared. Results : Compared with those in model group, histopathological scores of fibrosis, collagen content in the liver,liver fibrosis index and some indexs in serum were significantly lower, but liver tissue anti-oxidation index was siguificandy higher( P 〈 0. 05 - 0. 01 ) in prevention & treatment group of Amydae Carapax decoction. The effect of group of Amydae Carapax decoction was comparable to or better than that of γ - interferon and Fuzheng Huayu capsule, generally accepted effective anti-liver fibrosis drugs, the effect in prevention group and high dose group was particularly good. Conclusion:Amydae Carapax decoction had significant preventive and therapeutic effect on liver fibrosis. Its mechanism was related with resisting lipid perioxidation,improving pathological function of liver tissue,improving liver function, regulating cell factor level and regulating signaling pathway. Thus it could inhibit hepatic steilate cells (HSC) activated multiplication and extra cellular matrix (ECM)synthesis/secretion, promote ECM degradation and siguificantly block/treat liver fibrosis.
