职业性三氯乙烯药疹样皮炎住院患者发热原因及护理对策

摘　　要：目的探讨职业性三氯乙烯(TCE)药疹样皮炎住院患者发热的原因和护理对策。方法回顾调查医院2000年1月-2006年12月职业性TCE药疹样皮炎住院发热患者的有关临床资料,分析患者发热的原因和总结正确的降温及护理措施。结果82例职业性TCE药疹样皮炎住院发热患者中,有18例出现≥2次的发热。因感染因素发热的54例次,其中以皮肤黏膜感染为主共22例次,肺部感染18例次,上呼吸道感染10例次,菌血症和败血症共4例次;非感染因素发热64例次,其中疾病因素39例次,药物过敏反应16例次,其他因素9例次;发热的热程<7 d 102例次,>7 d 16例次;发热热型以不规则热、弛张热和稽留热多见,其次是间歇热,热型的表现与发热的原因未呈现明显的特异性。结论通过对职业性TCE药疹样皮炎住院发热患者体温变化特点、细致观察病情和及时协助做好相关检查,可为职业性TCE药疹样皮炎住院发热患者早期原因诊断及选择有效的降温和护理措施提供依据。OBJECTIVE To explore the fever causes and nursing countermeasures for inpatients with occupational trichloroethylene （TCE） drug eruption-like dermatitis. METHODS The related clinic materials of occupational TCE drug eruption-like dermatitis inpatients with fever in our hospital in Jan 2000 Dec 2006, were investigated and analyze the fever causes of patients to conclude the correct cooling and nursing measures. RESULTS Of 82 occupational TCE drug eruption-like dermatitis inpatients with fever, 18 cases showed twice or more times of fevers. Among 54 cases -times with fever caused by infections, 22 were mainly by derma and mucosa infection, 18 by lung pulmonary infection, 10 by upper respiratory tract infection, and 4 by bacteremia and sepsis. The fevers of 64 cases-times were caused by non-infection factors, of which 39 were by diseases, 16 by drug allergies and 9 by other factors. The fever periods of 102 cases-times were fewer than 7 days and those of 16 cases-times were over 7 days; most of fever types were irregular fever or continued fever, and intermittent fever followed. The types and causes of fever showed no obvious specificity. CONCLUSIONS The analysis on the features of body temperature changes, careful observation on the ill state and help in doing related checks timely can provide basis for the early causal diagnosis of occupational TCE drug eruption-like dermatitis inpatients with fever and the choice of effective cooling and nursing measures.
