摘  要:目的:探讨多种磁共振(MR)扫描序列对关节软骨病变的诊断价值。方法:应用7种MR序列组合对29例疑诊关节软骨损伤患者的30个膝关节进行检查。MR序列为:附加脂肪饱和的T1加权、T2加权及质子密度加权快速涡流自旋回波序列(TSE-T1WI-SPIR、TSE-T2WI-SPIR、TSE-PDWI-SPIR),分别附加脂肪饱和、水激励、磁化传递的三维快速梯度回波序列(3D-FFE-SPIR、3D-FFE-WATS、3D-FFE-MTC),附加水激励的三维平衡快速梯度回波序列(3D-BFFE-WATS)。分析各序列中病变软骨的表现形式,评价其对于软骨病变的诊断价值。结果:30个膝关节中,检出骨关节炎13个,剥脱性骨软骨炎2个,软骨挫伤6个,软骨未见异常9个。3D-FFE-SPIR发现损伤最多为45处,其次为3D-FFE-WATS(39处)和3D-BFFE-WATS(33处),TSE-T2WI-SPIR(31处),TSE-PDWI-SPIR(31处),TSE-T1WI-SPIR(23处),最少为3D-FFE-MTC(16处)。结论:3D-FFE-SPIR为目前显示关节透明软骨和软骨病变的最敏感扫描序列,水激励具有明显优势,可以取代常规频谱预饱和反转恢复技术用于常规临床检查。BFFE-WATS对软骨病变检出能力与TSE-T2WI-SPIR、TSE-PDWI-SPIR相当,稍逊于3D-FFE-WATS,但可清晰显示关节积液,其扫描速度快,在匀场良好的前提下可以应用于临床。Objective:To investigate the value of various magnetic resonance (MR)imaging sequences for the articular cartilage disease.Methods:MR sagittal scanning of knee joints were performed in twenty-nine patients who were suspected having cartilage injury.MR scan sequences were TSE-T1WI-SPIR,TSE-T2WI-SPIR,TSE- PDWI-SPIR,3D-FFE-SPIR,3D-FFE-WATS,3D-FFE-MTC,3D-BFFE-WATS.The image characteristics and the detecting ability of every sequence on cartilage lesions were analyzed.Results:There were 13 osteoarthritis,2 osteochondritis,6 traumata and 9 normal in the thirty knee joints.The number of injuries were detected as follow: 3D-FFE-SPIR:45,3D-FFE-WATS:39,3D-BFFE-WATS:33, TSE-T2WI-SPIR:31, TSE-PDWI-SPIR:31, TSE- T1WI-SPIR:23,3D-FFE-MTC:16.Conclusion:3D fast field echo sequence which attached fat suppress tech- nique is the most sensitive sequences in showing cartilage injury.WATS can substitute SPIR.3D-BFFE-WATS's ability for finding cartilage damage which is equal to that of TSE-T1-SPIR,TSE-PDWI-SPIR and a little inferior to that of 3D-FFE-WATS,but effusion is showed clearly.It has high scan speed and can be used in clinical cartilage examination.
