摘  要:目的:探讨免疫球蛋白E(IgE)的表达、嗜酸性粒细胞(EO S)浸润程度在鼻息肉、鼻息肉病中的差异。方法:应用连续两步免疫酶法(Sandw ich)检测鼻息肉病16例、鼻息肉31例和正常下鼻甲黏膜11例组织匀浆中IgE的表达水平;同时应用Chrom otrope 2R特染法检测以上组织中EO S的浸润程度;血液自动分析仪计数外周血EO S。结果:(1)鼻息肉病、鼻息肉、正常下鼻甲黏膜三组组织中IgE的表达水平不同,两两比较差别均有统计学意义(P<0.01);且其水平分别与鼻息肉及鼻息肉病组织中EO S浸润数呈正相关(P<0.01)。(2)三组组织染色切片EO S计数统计学上不全相同,两两比较差别均有统计学意义(P<0.01)。(3)血EO S数在三组之间的差别均无统计学意义(P>0.01)。结论:IgE对鼻息肉及鼻息肉病组织中EO S的聚集具有重要作用;IgE的半定量和鼻息肉组织中EO S计数可以作为区分鼻息肉病或鼻息肉的参考指标。鼻息肉病的高复发可能和EO S的高浸润程度、IgE的高表达水平有关。Objective:To study the differences of immunoglobulin E (IgE) and eosinophil (EOS) in nasal polyps (NP) and nasal polyposis. Method :Nasal polyposis (n = 16) ,nasal polyps (n = 31 ) and healthy nasal mucosa samples (n = 11) were taken during routine endonasal surgeries. The tissue bound IgE was measured with a sequential two-step immunoenzymatic (Sandwich) assay (Access immunoassay system, Beckman, USA). The degree of eosinophil infiltration was determined by Chromotrope 2R histochemical method in the corresponding tissue. The number of EOS of blood was counted in each group. Result:There were more significant differences between either two groups about the level of tissue bound IgE and the degree of eosinophil infiltration. There was a significant correlation between tissue IgE and the number of eosinophils in the nasal polyposis group and NP group(r=0.62,0.54,P〈0.01). There wasn't a significant difference about the number of EOS of blood in each group. Conclusion:The tissue bound IgE plays an important role in the recruitment of eosinophils in NP and nasal polyposis. The degree of eosinophil infiltration and the level of tissue- bound IgE between nasal polyps and nasal polyposis is more significant differences ,which can be egarded as a diagnostic indicator for discriminating nasal polyps and nasal polyposis. Furthermore,high expression of IgE and high infiltration of EOS in nasal polyposis may be contributed to the recurrence of nasal polyposis at a high level.
