老年2型糖尿病骨密度、骨转换生化指标的变化及相关性研究

摘  要:目的:探讨老年2型糖尿病患者骨密度变化及其与各种骨转换因子的相关关系。方法:采用单能定量CT(QCT)对53例2型糖尿病患者及40例健康老年人进行脊柱骨密度(BMD)测定,并分析受试者的骨密度与骨钙素(BGP)、骨特异性碱性磷酸酶(B-ALP)、抗酒石酸酸性磷酸酶-5b(TRACP-5b)、性激素等指标间的关系。结果:⑴老年2型糖尿病患者腰椎BMD均值显著低于对照组〔(74.75±22.70)mg/cm3vs(88.04±28.29)mg/cm3,P<0.05〕;老年2型糖尿病组B-ALP及TRACP-5b均显著低于对照组〔(21.59±5.31)ng/ml vs(23.98±4.61)ng/ml;(2.89±0.75)u/L vs(3.30±0.79)u/L,P<0.05〕;⑵受试者BMD与骨转换生化指标间无显著相关性。结论:老年2型糖尿病患者存在不同程度骨密度下降,且骨形成与骨吸收功能均降低。骨转换生化指标和骨密度是两个相互独立的检测指标。Objective:To investigate the changes of bone mineral density(BMD) and the relationship of BMD to biomakers of bone turnover in the elderly subjects with Type 2 diabetes mellitus(T2DM). Methods:BMDs of spine were measured by QCT in patients with T2DM (n = 53 ) and health controls (n = 40). The relationship of BMD with serum bone Gla protein, bone alkaline phosphatase, tartrate resistant acid phosphatase 5b levels and sex hormone were also analyzed. Results:(1) The spine BMD values in the T2DM patients were significantly reduced compared with control [ (74. 75 ± 22. 70 ) mg/cc vs ( 88.04 ± 28.29 ) rag/cc, P 〈 0.05 ) and the concentrations of serum bone alkaline phosphatase, tartrate resistant acid phosphatase 5b in T2DM group were significantly lower than controls [ ( 21.59 ± 5.31 ) ng/ml vs ( 23.98 ± 4. 61 ) ng/ml ; ( 2. 89 ± 0. 75 ) u/L vs ( 3.30 ± 0. 79 ) u/L, P 〈 0. 05 ] ;(2) The bone density of subjects have nonsignificant relationship with biomakers of bone turnover. Conclusion :The spine BMD in elderly T2DM patients is lower than that of control group. And the function of bone formation and bone resorption all degrade in elderly T2DM patients. Biomakers of bone turnover and bone density are interdependent detection indexes.
